Breastfeeding

Another spitting up question...

My baby is 8 days old and we had a rough start to nursing due to his severe tongue tie.  That's all resolved now, but we have developed a new problem over the past few days...projectile vomiting!  He does this twice a day, with his first daylight feeding in the am, and then again in the evening/early nighttime.  An LC was here yesterday for a follow up and did a weighted feed...DS took 2 1/2 oz in about 15 min in the am!  Suggestions?  I'm wondering if my let down might just be a little much for him with all the extra milk during that first feed especially.  I'm wondering if maybe pumping a little first would help...I don't want to mess with my supply since we are less than 2 weeks pp, but he's already basically feeding 2 or 3 times with that feed ( he nurses again after he vomits because his belly is empty again), and I was planning to add a pumping session at that time around 4 weeks anyway.  I have no clue what I would do in the evening since it's not as regular as the am one, but at least that woud cut the extra puke laundry in half!  What do you ladies think?

  • It could be a hindmilk/foremilk imbalance. I dealt with this about a week ago. I was dealing with a crazy oversupply so DS was only getting a ton of foremilk which is high in lactose and fat. This is usually great for them and exactly what they need; however if they are not getting the hindmilk it becomes too much lactose for them to digest and causes spitup/gas problems. I would suggest pumping before hand if you feel this is the problem. After I started doing this I decreased my pump time by one minute every day to decrease my supply and now DS is able to finish off one boob at each feeding and gets the hindmilk. This problem came in addition with green poop for me, so check that maybe?

    Is it spit-up or projectile vomiting?  If it is projectile vomiting, that could be more serious...but if it is just spit-up, i know that my LO started spitting up after every feeding not too long ago and she was just taking in too much at one time.  I have to stop and burp her a lot more frequently now, so you might want to make your LO take more breaks during the feeding to burp.  And like you said, pumping beforehand might help, too.  Also, your LO is such a cutie!

    It definately is projectile, but because of the timing, I'm really not worried about other issues at this point (well, like any new mom I AM worried, but I'm trying not to be neurotic!). He is not dehydrated and has been peeing and pooping tons.  He is also a horrible burper, which is why I'm less inclined to believe it is a more serious problem at this point. I should tell you, I'm a NICU RN...I know a ton about babies, and a ton about breastfeeding, but I certainly don't know everything there is to know!  I wish I didn't know as much as I do some days, being a new mom is hard enough without having to worry about crazy things that most new moms don't even know exist!

    This makes sense to me, and I did know about the foremilk/green poop connection...no green poop so far.  Thanks for the tip about decreasing pump time though...if we go that route I'll keep that in mind!
