Post birth- one twin not gaining

I have twin frat boys that are 1 month old and A is gaining but B is not. They are both BF and supplemented with bottles of pumped BM after nursing. We did weighed feeldings w the LC and they both took the same amt of mik. He nurses well and doesn't down his supp bottle so he seems not hungry. Yet he has only gained 2 oz in over 2 weeks. Twin A has gained 13oz in 2 weeks and I pump extra milk so it's not a supply issue 

The Ped wants me to add powdered formula to his supp bottle to make it more calorie dense. 

Just wondering if anyone has experienced this and what the cause was? Also what worked to help the baby gain weight?

  • My fraternal twin boys are a month old today. I have been breast feeding twin A from the beginning. Twin B was taken to NICU for low blood sugar they started him on formula there. I started nursing Twin B a few days after we got home but continued to supplement him with formula per NICU instructions. At our 2 week checkup he gained faster than twin A. After this appt, I started supplementing formula after each nursing for twin A as well now they are only 2 oz apart. I have used nipple shields from the beginning can't get my boys to nurse without them so that could be our issue but I really don't think it is because they seem to get a lot of milk when they nurse. Either way, supplementing formula has worked for us.

  • That's so funny we had the same due date and our boys are the same age!  Ok, glad to hear the formula worked!

  • Our little girl is still on Neosure preemie formula because she hasn't been gaining as quickly as our boy.  She isn't even on the percentage scale yet.  She is almost 9 pounds.  She just doesn't eat as fast as our boy and gets full quicker.  

    He is a bottomless pit.

  • Do you want to add formula to the bottle?  My twins gained weight very slowly when they were first born.  I was exclusively BFing and by 3 wks, they still hadn't regained their birthweight.  At that point, my doctor was "concerned" but did not force formula on them.  Her view was that they "were gaining slowly, but were still gaining" so it wasn't necessary to introduce formula at that point - they would continue to monitor them.  I did decide, on my own, to formula feed after that because I didn't know if there was a problem with my milk supply or their ability to BF. 

    Since one twin is gaining, and you're offering a bottle after that, its obviously not a problem with your supply - it's a matter of how twin B's body is absorbing/using it.  He might not be gaining weight, but is he growing in length?  We had an issue around 3 months w/ DD1 where she barely gained any weight (also BFing) between visits, but shot way up in length that month, because her body was using the food to grow longer, not heavier. 

    If it were me, and I didn't want to add formula, I'd wait another week or two and have baby B re-weighed, because while he's not growing as fast as his brother, he is still growing.  That being said, there's absolutely nothing wrong with supplementing with formula if you are at all concerned.
