appt update

Well, the cnm agreed with me that determining chorionicity is important. Not important enough to.do do much about it in any sort of hurry though. The mfm next door does the u/s and they referred me for an u/s appointment for probably next week.or the week after that. And maybe just because she is not the ob, but i didn't like some.of the answers she gave me. She did say they let you go.full term if you get there (and she meant regular full term of 40w) and nothing about nst. I really want all my care to go to a mfm entirely, but they seem to be a bit like Oz the Great and Terrible in that it is a freaki g impossibility. I even showed her my earliest u/s pic whrre they are clearly in one sac, but she said you could not tell anything from that. I don't know what to.at this point other than move to a different state. At least i will eventually get an u/s to determine chorionicity. I really do not.know what i can possibly do. I am.actually contemplating faking an emergency...although i really really reLly disagree with that ob so many levels....i

  • Geeze it's tough to get some reassurance out there, isn't it... Do you think that you could bring up your frustrations with the MFM when you go in for your ultrasound and see if you can convince her to take you on for your primary prenatal?

  • That seems to be the only optio i am.left with (although i am.about to.call the insurance company to see how i can.somehow get to a mfm without a referral since these drs all think they are God and that i don't actually need to.see a.mfm other than for the a/s and not before that at all. I am going to atart making some calls too to see if i can find an out of state mfm that will see me.without a referral. Also, unrelated, but i decided i don't like the cnm i saw. She told me i should only gain 20# for the entire pregnancy b/c i am so small (i am 4'11", and gained 25ish with both.of the singletons....the majority of it at the very end. I have been struggling to try and gain weight early for the twins...i am up2-3. #s which i.am.thrilled.about) i get the feeling yet again, that i am.dealing with people who.don't know what they are talking about. 20 pounds.for twins...that aounds good.and healthy, right?
  • I am so sorry you are having to deal with all of that!!! HAve you read Dr. Luke's book by any chance? I highly recommend it, especially given the fact that it seems like you are going to have to bear the brunt of your own prenatal care, since your doctors kinda sound like idiots.

20lbs sounds like absolute bull to me. My own personal goal was to basically gain a pound a week, and I think I remember Dr. Luke being along those same lines (24 lbs by week 24 or something). Also, she says that your goal is to put on the weight earlier on, because you need to build up some reserves for down the road when your stomach has absolutely no room for food, etc.

Anyway, I really recommend that book. I have an awesome OB who never let me down, and I still thought it was a great resource.

    Hang in there!

Yes, i got the book as soon as i heard of it after finding out there were twins. I think the weight gain she recommends is 20# by 20w (or maybe 24 by 24?....either way, it is a heck.of a lot more than 20 total). If it were not for the book and these boards i would have no.clue as to the shoddy care i have received thus far. I refer to it often (but it is impossible to make actual medical professionals take me serious despite me telling them what i have learned). Sounds like bull to me too...i was just pointing it out because it blatantly seems.like really bad advice...another indication i am in DIRE need of someone who either knows what they are.doi g or is willing to.pick up aome reading material.and learn.

That's what I did, but not with my first MFM, who I didn't like. But at 22 weeks, I switched. Hopefully you can too.

  • Just speaking from my experience here, but I'm 4 foot 10, thinner side of average. My OB and peris wanted me to gain 35 to 50 lbs during my pregnancy. I struggled to gain partially due to gestational diabetes diet and finally weighed in at 35 lbs gained the day before I delivered 36w0d.

  • So.....apparently the standard of care for Maine is no mfm until 20 weeks without adequate medical reasons. (and monochorionic does not count). I just talked to a nurse at a totally different prwctice and this is what she told me. My chorionicity u/s is next week, with no mfm involved (i am waiting on a call back as to whether i can see him at this time or not. I strongly anticipate being denied). I suppose i have an actual real life consult with a peri at 20 weeks, so i am going to have to settle.for that and hope the babies.don't get early onset ttts in the meantime. Sigh...this is.so hard.
  • Does the OB have you scheduled for regular ultrasounds (once every 3-4 weeks) until your 20 week referral? That should greatly aid in catching TTTS if it happens, because they'll be able to look at growth discordance. I don't know as much since my twins are di-di and I didn't find out about them until 20 weeks, but the OB keeps an eye on how they're growing at my regular ultrasounds, and I don't need to see an MFM. Because yours are most likely mono-di an MFM is important, but it sounds like many of the MoMs on here didn't start going until 20 weeks, so hopefully your little guys will grow like champs until your referral!

i am not sure. I just talked to the cnm i saw today (referral DENIED...anyone surprised?)(she was very obviously annoyed with me too)...the chorionicity u/s next week will be reviewed by a peri and i will be co tacted as needed (she does not think.i will be, since mono-di twins are not actually really high risk.)(her words)(assuming i.am even.mono-di, again her words). So...si asked what steps will be taken to.ensure they don't get ttts (told i spent too much time reading online) and she said regular monitoring of growth. I don't have another appointment for.a.montb with these people...she seems to believe 1x a month mo itoring is adequTe...i said something snarky about her babies being the dead ones o. The wrong side.of the statistica...we are developing a really great long term.relationship here...huh? There is literally nothing else i.can.do.other than hope she is right and right letters requesting earlier appointments and possibly moving in.with my mil.in another stat which i am seriously.considering. other than that, cry..,,.,........... Seriously though, am i being unreasonable? Honestly....i do understand my posts are.long and complainjng and annoying, but i.would like honest opinions from other MoMs as to whether i am wrong to want chorionicity to be established by now and to see a mfm for a consult prior to 20w. From the very beginning, i feel like this has been treated as an unreasonable request...Maybe i am a little fraught with anxiety...skewing my.perceptions. honestly...unreasonable or totslly justifies? In between somewhag? Thank.you thank.you thank.you.....
  • OMFG only 20#s??? The hell kind of training does the woman have?? I'm borderline obese and I'm supposed to gain 25-30 this go around, as per both my Peri & OB... I gained 20# with my singletons when I was a healthier weight. Every doctor I've spoken with says that small to healthy weight women should gain 35-50lbs with a multiples pregnancy because they'll have baby weight, water weight, and gained fat if they are eating as many calories as they should. You need to move to Oregon and hook up with my doctors lmao

  • Luckily i am somewhat educated and i completely realize her advice was poppycock. I plan to gain as much (healthy)weight as i can. I am not going to be thrilled if/when i am chastised for getting too big, but i will know deep inside that i am right and she is wrong so i can take it. I think dr. Luke is insane, but i am trying to eats lots of dairy and meat and healthy things and do the best i can. I am so ao so glad i got the book though, because it is more instruction than "eat healthy". I am on my own until at least 20 weeks....hopefully the mfm is decent...at this point all i.am hoping for is decent. I may poop myself if he turns out to be good! I won't know for another month!!!!
  • We found out at 6w5d the girls were mo/mo. I had u/s every 2 weeks from then on, franticly hoping to find a membrane. While mo/do is HIGH RISK, that lady is nuts! mo/mo is even more high risk, do not google it! But at 16 weeks we found the membrane. Its my understanding it is easier to determine early on before everything gets all cluttered in there. I stuck with my ob because it was an hour away, he also did a few years on a multiples floor while doing residency. I saw a mfm 3 hours away at 20 weeks, but we had a great pregnancy no complications or anything so we didn't need the mfm and luckily my doc knew what he was doing. Its not worth the stress to stick with someone that makes you so worried and doesn't have straight answers. Pregnancy is no joke and multiples is even less of a joke! As far as weight goes don't get too stuck on it. I actually lost weight at first and only gained 3lbs by 20 weeks, definitely put on more after that! But my girls were healthy 6lb14oz and 6lb7oz! GL hope everything works out for you soon!!
