I am BFing and breast milk can sit at room tempeture for a while so I do one of two things. If I am going somewhere I can't BF or warm a bottle I take the bottle at room temp. If I am at a resturant I ask for a cup of hot water.
We have DD used to room temp (sometimes even cool bottles), and it is one of the greatest things we have done. If your LO has to have warm bottles, the make travel bottle warmers that plug into your car. Or they make these heating packs which are basically the opposite of the freezer packs you use to keep things cool.
We have a bottle warmer and bring along a thermos filled with boiling water that we can throw in the warmer (if there's nowhere to plug the warmer in). It's kind of complicated, but it works!

We store bottles in a lunch bag that is meant to keep cold things cold (can't think of the term for that right now...). This way, even if we didn't use the bottle while out, we can still use it later, because it's maintaining it's refrigerated temp.
We also carry a plastic container that we fill with whatever warm/hot water we can find. Sometimes it's warm tap water, sometimes it's water that gets microwaved to a boil, sometimes it's boiling water at a restaurant. Then we put the bottle in the container for 30-60 seconds to warm up (the same we do at home).
I used to do a cup of warm water when we were at a friends or restaurant. Just plop the bottle in and let it warm up. If we were on a road trip then we used an automobile bottle warmer that someone gave us at our shower.
