One Sac and Two Yolks at 5 weeks!?

I'm a regular poster on the IF board. I have a question that I thought maybe moms of multiples could help me with.

Just completed my first IVF.  Had my first ultrasound today (5 weeks, 3 days). Very early to tell much, but the doctor said we have one sac with two yolks inside.

He said that it looks like the embryo split (we did ICSI and assisted hatching). He is having me come back in one week to see if fetal poles develop.  He also mentioned that if 2 fetal poles develop, the next hurdle is to make sure they have a membrane separating the two or else it could be a very high risk pregnancy.

MH and I are in shock about the possibility of identical twins. And concerned about the risk of losing one or both and also of not forming a membrane.

I'm curious if anyone started out the same way? Any advice is greatly appreciated. Maybe I'll be joining this board some day!

  • I never had IVF but there is a chance that you could end up with mono/mono or mono/di twins.  mono/mono is the higher of the two because the twins would share a sac and a placenta.  If they are mono/di then they would only be sharing a placenta and have their own sac.   Of course this early on you could loose one due to Disappearing Twin Syndrome but lets not think of that.  Try ( yea i know .. ya right) to relax and take it easy.  Shock is VERY normal.  When I found out we were going to have twins at 7 weeks I could have fallen off of the table.

  • I wouldn't stress out about the membrane just yet.  Lots of mamas with mono/di pregnancies who have early u/s aren't able to see the dividing membrane until the 2nd trimester.  The membrane is always there but since it's so thin, sometimes it can be hard to see on u/s.  Like PP said, mo/mo twins are very rare.  It definitely takes some time to sink in.  Good luck!

  • Congrats!! The same thing happened to us. My RE hadn't seen ID twins in several years, so we were all surprised. I think it's pretty awesome :) Chances are everything will be just fine, so enjoy every minute and sleep while you can!!

  • I was told mono/mono and recently found out mono/di so I know the feeling!! It was hard not to worry but I tried to just stay positive! We found out at 6w3d (twins and we thought mono/mono) so still early and we've made it this far happy and healthy! Keep us updated!!!
  • I was told they couldn't find the separating membrane and we might have mono/mono twins. However, to our relief, they could see the membrane at 12 weeks and we are now expecting mono/di twins. It's still high risk but much less so than MoMo twins. Hang in there for a bit longer.
  • Yep, mono/di girls here and at 6 and 7 weeks they were worried about mono/mono because they could not see the dividing membrane. Then at 12w at the specialist there it was, all safe and sound!  GL!
