DURING LABOR
- I would prefer to remain as mobile as possible, including:
- a Hep Lock instead of IV unless fluids become necessary
- intermittent monitoring unless constant monitoring becomes necessary
- being encouraged to labor in various positions and try various coping techniques (birth ball, shower/tub, waling, breathing techniques, visualization, etc.)
- Please do not offer medication/epidural for pain management (I will ask for it if desired)
- Frequent internal exams may be declined; please leave water and membranes intact
- Please support the perineum during pushing / crowning to minimize tearing
- I would prefer to tear naturally (no episiotomy) unless necessary in an emergency situation
- Local anesthesia to repair tearing is permitted
- Avoid the use of forceps or vacuum during delivery
- Avoid a cesarean section at all costs unless necessary for baby?s safety (please attempt manual methods if baby?s position needs to be changed for vaginal delivery) (if surgery becomes necessary, husband is to be present)
- Please delay clamping/cutting the cord until it has stopped pulsating
AFTER BIRTH / BABY CARE
- I would prefer immediate extended skin-to-skin contact with baby prior to weighing/assessments
- I would like to attempt to nurse immediately following delivery with support of a lactation consultant
- Please perform routine procedures in the room and discuss them before being performed
- Baby is to be exclusively breastfed / offered expressed breast milk (no formula, sugar water, pacifier)
- Baby is to room in with parents
- First bath is to be performed in room with assistance / involvement of parents
- If C section becomes medically necessary or baby needs to be removed from the room for further assessments/treatments, father is to stay with baby at all times

Looks good. Clear, short.
Some of the after birth things are standard where I gave birth (rooming in, bath in room), so you may be able to shorten it even more after you discuss with your care provider.
