Mono/di twins & vaginal delivery?

 My OB has told me from the beginning that I will need a c/s because of the risk of TTTS during labor. I'm a little disappointed because I have a spinal injury that prevents me from receiving a spinal or epidural, which means general anesthesia for me. 

 Have any of you ladies delivered your mono/di twins vaginally?

  • My friend just had mono/di twins and she had to have a c-section as well for the same reason.

  • im pretty sure the other reason the dont let you deliver vaginally with twins who share a placenta is the serious risk of the placenta separating after the first one is born before the second makes it out, increasing risks of oxygen deprivision substantially.

  • You can absolutely deliver your mono/di twins vaginally!!!! I did, and I had what everyone in the hospital called a textbook delivery, no problems whatsoever. I did deliver in the OR and was monitored externally and internally from the time I was admitted....

    Yes, there is a risk of accute TTTS during labor, but that's why they monitor both babies continuously. I always knew there was a chance I could end up with an emergency c-section, but it was a risk I was willing to take, and my OB was supportive.

    If you really want to try for a vaginal delivery, I say go for it! As long as they are monitoring both babies, they can get them out via c-section in no time, should one of them go into distress.

    FWIW, my OB did recommend I be induced at 37 weeks b/c of the risks of placental deterioration any later than that. My boys were born at 37w1d and had no NICU time, we all went home the next day!

    Very scary, especially since I cannot receive spinal or epi. I could put 2nd baby at greater risks waiting to be put under general if c/s  for 2nd needed.

    That is a risk that I didn't think about... because I had an epidural, doing an emergency c-section wouldn't have been a big deal, but waiting for the general to kick in probably would take too much time.

    In your case, the c-section is probably the safer route all around. Either way, what counts is bringing home two healthy babies and a healthy mama!

    Ah, that is wonderful. My dream twin delivery haha. It is sounding like I wouldn't even be a candidate for vaginal though with my spinal injury. If they had to do emergency c/s, they would have to put me under first. Sounds too risky. Gah.

  • Very true. I'm actually feeling more comfortable with the c-section now that I realize it is probably safer. You know sometimes you just need to write it out/say it for things to make sense.

  • I'm having mono/di twins and my docs are pretty serious about my having a vaginal delivery/ with no episiotomy. There is, of course, the possiblity of complications for any twin delivery but that is why I'm goin in the OR. I REALLY hope they both come out, no C sec, safe and sound... no episiotomy, no tearing. Everything else has been ideal, why not shoot for the stars now!

     I'm realistic, but... It's 100% possible, even if its not probable. 

  • My MFM has made it clear that they prefer a C Sec for our mono/di girls due to risks and my health complications,however I haven't outright been told that vaginal isn't an option. I plan to bring it up soon.

    I did! I was induced at 37w and they were both head down so we went for it and it worked out great! My doctor had been monitoring me weekly for TTTS and IUGR of Baby B, and I delivered in the OR in case any complications arose that required emergency csec, but the birth was pretty perfect!

    If they give you the option for vaginal then they must be pretty confident they can get you under anaethesia in a quick enough manner to prevent any harm in an emergency. I would discuss your concerns at your next appt (or call the office if you're not due for an appt soon) so you know beforehand what your options and risks are.

  • I had mono/di twins and a vaginal delivery.  They both turned head down super late but they induced me at 37 weeks and it was fine.  I did have an epi but the Dr said it was my choice they wouldn't force it.  In fact, I had a midwife deliver though the Dr was there too.

    I'm not sure that the no epi makes it harder to go to an emergency c/s.  Putting you out with general is actually faster.  I would talk through it again with your Dr and really listen to him/her.  If the Dr isn't comfortable with attempting a vaginal delivery I just wouldn't do it. In the end - two safe babies is the ultimate goal.  I just found after talking through what I wanted with my Dr they were open to way more than I realized.

  • I delivered my mono/di twins vaginally.  It can be done.  A lot of doctors don't like to do it though and push c-sections on their patients.  Honestly, I wanted a csection at first but my doctor explained why a vaginally delivery was best and after weighing all my options I Went for it.

    You cannot judge need for episiotomy prior to delivery.  That's just a weird thing for your doctors to say.  I rarely ever cut them, but there are times when you do need one.

  • I delivered my mono/di twins vaginally.  Neither my MFM nor the OB who delivered me said anything about needing to do a c-section because they were mono/di.

  • My boys were diagnosed with TTTS at 16 weeks, is your doctor saying this is something that can happen during labor? I thought the continous monitoring during preganancy would let you know well before you deliver, if your babies have it? Either way, I had laser surgery at 17 weeks, no further issues, was induced and delivered vaginally at 37 weeks and 1 day.
