Never as well, we have been using cloth since DD was 5 days old. Have you thought about using cloth, it is great, and if you ask any cloth Momma, I'm sure they will tell you they have had very very few if any.

At least once per week. DD takes huge poops everyday. We've tried BumGenius cloth, huggies, pampers, luvs, and are now in up&up. She's also in size 2 at just 11lbs. So far the up&up size 2 seem to be working better than anything else.
Usually once a week, though less lately. We use cloth but DD is tall and skinny and when she goes, it's an explosion :P so there's leakage a lot of times. But honestly never had a HUGE blowout... just leaking around her leg sometimes.
1 time and it was my own fault hehe We cloth diaper and I was in a hurry changing DS beause we were getting on a subway and it was about to leave. I didn't check to make sure there no gaps in the leg area because the bumgenious xsmall dipes, which I used, tend not to fit well in that area....big mistake. He took a dump and it shot out the gap in the leg area and it got all over my DH's pants...thank god it wasn't me
We had two in Pampers before we switched to cloth, since then I've only had one and like someone else said it was because of user error on my part. It was a new style diaper that I had just ordered and wasn't sure on the sizing/snaps so there was a pretty good gap that poop just "shot" out of.
I've had a lot. I use Pampers and will not once this box is done - I find Huggies work best for my LO. I'm just starting to cloth diaper and have had a few blow outs too - I must be doing something wrong b/c everyone says cloth is so much better.
We use Pampers Swaddlers, and have the occasional pee leak but a poo leak only once so far. We had frequent pee leaks when we were at the end of NB's that went away when we moved to size 1's.
We've also tried Target Up & Up as well as Huggies, and found the former to be just as good as Pampers, and the latter to leak like crazy. I think the key is the length of the fabric between the diaper and leg elastic - the longer it is, the less it will leak. When LO pees, the diaper puffs out a lot to absorb the liquid, so the longer fabric makes sure the diaper stays snug while expanded.
I hold him over a little potty chair as soon as I see "the face." Im sure its more comfortable for him and while it takes a bit of practice, clean up is a breeze and we have no stinky diapers.

I would never be able to use this technique cause DS never makes faces when he goes poop. I only know when he goes because his poops are loud and juicy lol my DH's aunt used that technique with her 4 kids successfully though.
Just one that I recall. I do, however, jump to change dd's diaper too quickly and get poo explosions because of that. I've used pamper swadlers for the first couple weeks and Luvs more recently.

All the time!! We have pooplosions and pee pee leaks. I got 2 big boxes of the Babies R Us premium diapers and they are the worst diapers EVER!!!!
It's been much better since I switched to the pampers swaddlers!

I could not agree more. I had these and I'm not buying them again. I don't like their wipes either.

But, whenever I give LO naked time or take her out of the bath, I inevitably get poop all over me. Every. Single. Time.

Never we are in Pampers size 1. She is a constant pooper a little every diaper change (about 10 poops a day)
With DD #1 we also used pampers and she had blowout a few times a week. I think it was because she only pooped once a day and they were huge poops!
