level 2 results normal bloodwork abnormal

We just had our 17 weeks scan today and everything was great.  Our measurements were all normal no soft markers no abnormalities seen.  I am thinking great then the doctor reminds me of our 1st trimester bloodwork which was abnormal.  My papp-a was very low but everything else was great, the NT measurement was actually a little less than average and they saw a nasal bone.  I was so happy with our ultrasound results until the doc started freaking me out about our previous bloodwork.  We had decided that after the bloodwork (quad screen) that if the ultrasound showed any abnormalities we would have an amnio but if everything was great we would not.  I strongly felt this way until the doc kept talking about the papp-a value and my increased risk of downs 1 in 23.  From everything I have read that my risk is actually better than this now since we saw a nasal bone, the nt measurement is normal and the detailed ultrasound was normal.  the doc sent us home to think about an amnio over the next 2 weeks.  It just took us so long to get pregnant I just can't see taking the risk of miscarriage with such good ultrasound results.

  • my dr. reallllly tried to pressure me into amnio (after 2 soft markers on our level 2 u/s), and I refused.  but that was a personal decision ( I don't care for invasive testing) Our baby was fine.  that isn't always the case tho.  You have to weigh your own feelings on what you would do IF... IF you find out you have a child with a disability...IF you feel safe having the amnio... etc...  Most women do have amnio now and most women are fine with it so really in the end it is up to you and what you feel will offer you the best possible happiness.  I am sorry you have to feel so worried, I hope the best for you and your baby!

  • I would ask your doctor about the materniT21 test - I had an elevated risk for downs (you can scroll down to my earlier posts on this board), and I did not want invasive testing. The MaterniT21 is a blood test - no risk to the baby - and the results are considered highly accurate - it is a relatively new alternative to the amnio (and in my opinion, the wave of the future); it can take up to 2 wks to come back (though mine was back in 8 days) but it was SO worth it for piece of mind - if you decide to have it, if it is offered in your area, you may have to pay some out of pocket depending on your insurance (we paid $235 out of pocket).
  • My little sister has low Papp-a in her current pregnancy and nothing else the matter. it is not all that uncommon, and so far, her pregnancy has been uneventful. If you want a little reassurance but don't want invasive testing, I would recommend getting the materniti21 blood test.

  • My bloodwork during my NT scan showed an increase of 1:41 for Downs. Everything else during the u/s was normal and there was a nasal bone. I ended up having the amnio and everything was fine. My Dr. has been doing amnios for 25 years so I felt very confident with the odds. It is scary and sucks that this fear is placed upon us. Good luck with your decision.

    This is exactly how I feel.  My DH and I have decided that if we have increased risk factors after the 2nd tri screens, we are going to do the Harmony blood test.  I really do not see why your doctor is pushing so hard for an amnio. Are they older and not on top of the new tests? From what you said, everything except your papp-a value was good.  This can be associated with an increased chance for pre-eclampsia and other issues, not just downs. I would quietly see if all of your results could be sent to some one else for a second opinion.
