Three weeks now... tried A&D, Desitin, Resinol, Anti Monkey butt, switched wipes and diapers....I just dont know!!! TIA!! for any suggestions. DD has been to MD and given Nystatin with no help!!
DS just had some over the weekend and aside from frequent changings (with wet, warm washcloths instead of wipes) and heavily applied Triple Paste, I gave him lukewarm soaks in a shallow bath w/ 2T baking soda in the a.m. and p.m. I also let him go diaperless as much as possible--we'd put him bare butt on an old towel in his wagon and go for long walks, or have him play in the yard. I think the air helped more than anything.
If not, my opinion, give the baby a bath every night, and if you can rinse him/her just with water after a poop do it. Go to the sink and rinse.
Keep him/her barebutt as much as possible, when he's at home. Diaper off. I know there is going to be a lot of wet spots . Also try desitin extra strength, and literally LATHER it on, like 1 cm thick. (That of course do before bedtime with a diaper), but just be patient. If you do all of those it will go away eventually.
Possible yeast rash. My son had one due to a yeast infection on my breast. Use 1 cup of water mixed with 1 tablespoon of white vinegar. Use just water to clean baby and air dry then dab vinegar solution on rash. Also let LO go diaper less when possible. My guys bottom was much better in a day or so. Lactation consultant advised me to try small amount of lotrimin on breast to get rid of yeast. Good luck!!!
Was the Nystatin the lotion or ointment? Sounds like you need the ointment, not the lotion or cream.
When LO had a bad rash like that, we found that if we alternated diaper changes with Nystatin ointment with the purple tub Desitin and powder, and actual jock itch cream with Desitin and powder, worked the best. His rash was better within a day, and gone in about a week. We also made a point to sneak into his room overnight and change his diaper - but I wouldn't make a point of doing that unless your LO is a heavy sleeper like mine!
Are you BFing? Is it possible that you have thrush/yeast and are passing it back and forth? This sounds like a yeast rash to me. However, I've recently had a couple of mom friends with babies that had strep rash on their bottoms. I'd go back to the pedi.

Somebody told me today about her daughter getting an acidic urine after eating apples. Call me crazy, but I've never heard of that. I too, have a baby that I couldn't shake the diaper rash.
Recently I switched to baby powder because I ran out of the creams and it was the only thing around. Her face was priceless when I shook that stuff on there, and so I kept doing it to make her laugh. Bam....no more diaper rash. However, I also have been feeding her less apple juice lately. I wonder if maybe she is sensitive to certain fruits? I'm keeping an eye on it from now on. Just saying, maybe your LO might be sensitive too and that's why it's hard to get rid of.

I second the baking soda bath idea. Have you tried coconut oil? I alternated coconut for daytime when I knew he wouldn't be in a wet diaper for long and desitin max at night for protection . It seemed to help. Good luck!

Try using Calmoseptine. Its a bit hard to find because some places categorize it under First Aid rather than in the baby products section. This is because its used for other things besides diaper rash. Here's a link to find out more about it.
I use this if my baby has a particularly bad rash and stop using it once its gone and start using her normal diaper rash cream again.
Also try washing your baby's bum instead of using wipes and dry it out with a towel. I find that more gentle than rubbing wipes or anything else on their bums.
If you use disposable diapers. Consider switching to Huggies Pure and Natural. So far its THE SOFTEST disposable diaper I've used yet. And its not supposed to have any perfumes in it and its organic.

DS just had a yeast infection for 2 weeks and we tried the Nystatin and it didn't work either. We finally went back and they gave us another combo cream or ointment and it has just about went away in less than 2 days. I wouldn't try the doc again or like pp said air dry works good too. GL
I had the same situation as you too. It takes my mother-in-law to clear it out. She rinse my baby's butt in the sink with warm and light soap after every poop or diaper change. It was awkward for me to do it at first and very cumbersome, but after a few practice, u learn how to hold the baby with one hand and splash warm water, rub his butt and apply soap with the other hand and run it under the bathroom/kitchen sink. Apply some cornstarch baby powder before putting on diaper. She said the enzyme from the poop and the ammonia from urine irritate the skin and must be wash away, wiping it with wipes will only rub it in even more and make it worst. I still use hydrocortisone while the butt is still red, after 2 days of frequent washing, the butt does look better and I stop using the hydrocortisone. Until this day I get into the habit of washing butt after each poop, (not with the wet diaper) plus bath him every night.
Go old fashion for can't lose results cornstarch is best along with air drying simply put your baby in crib or bassinet naked after applying cornstarch I have seven kids believe me this works. Also this is where it gets icky try no wipes either clean baby wash cloth white color only or simply use warm water to wash babies genital area some babies can't handle even sensitive baby wipes another thing is new mothers always go for the name brand diapers try rite aid or target or babies r us store brand diapers they work good and are usually biodegradable

Mix together Maaloxx and Aquaphor (equal parts) and then apply that as a paste.
My friend who is a nurse told me this one and it works like a charm.
