We were just prescribed an epi-pen today at my request because DS has broken out in whole-body hives on 2 occasions so far in response to different foods. (I know for sure what caused one of the reactions, but not the other).
At home I'd just use my best judgment, but daycare requires me to fill out a form stating specifically when DS should receive the epi-pen injection. This is what I have so far:
"Administer upon difficulty breathing, swelling of the lips or tongue, unconsciousness, weak pulse or fast heartbeat in conjunction with wheezing or breathing difficulties."
Does anyone think I should add anything? I really wish they could just use common sense.... I don't want them to administer it if he's having trouble breathing because he has a stuffy nose, for instance. But I understand their position as well.

I would take out "in conjunction" - that somewhat indicates that he would have to have BOTH a weak pulse/fast heartbeat AND wheezing/breathing difficulties. I would want them to administer it for any one of those symptoms. You might add something about severe hives.
This. An allergy action plan spells out exactly what to do when different symptoms present. It takes the guess work out of it which should be a comfort to you and your provider.

Does your allergist provide you with a written action plan? Is this something you specifically requested? Ours just gave us a general overview of when we would or would not use it.
Hmmm....I think our allergist gave it to us without asking. I think most allergists ask if your child is in school or a child care setting. If so, they usually offer it up because most schools/daycares require it.

His allergist is terrible. DS tested negative for everything, so the allergist said I could give him any foods I wanted and he didn't need an epi-pen. His pedi prescribed an epi-pen at my request.
DS sees a new allergist in a couple of weeks. Hopefully they'll give us a plan.
This. Our allergist today said 'Never be afraid to give an epi, be afraid NOT to.' He gave us a list of things to watch for.
