Best way to combine Cold milk

When you refrigerate milk it separates. When i try to combine it I want a mixture of the cream with the milk, but I don't get it off the sides completely when I swirl it. I'm trying to figure out the best way to combine cold milk to cold milk. Any ideas?

I hold the bottle in my hand for a minute before I swirl it. It's just enough warmth to get the fat to mix back in the milk without getting it too warm.

?? Are you trying to combine refridge BM with refridge cows milk?

I just shake the heck out of the BM to get the milk off the sides, then put it in a bottle with WCM, shake again, and than put the bottle in warm water to heat.

I'm usually impatient when I'm trying to combine cold bottles from the fridge. I found that if I vigorously rub the outside of the bottle with my hand where the milk fat is stuck to the sides for a few seconds, the friction creates enough heat to quickly dislodge most of it. And then if I swirl a few times, the rest usually comes off fairly easily. Otherwise, I just let it sit on the counter for a few minutes and it usually warms up enough to loosen most of it with just a few quick swirls.
