Coming Over from Dec 2012 - Vaginal Breech?

Hi ladies!

H and I are due in December and are planning a med-free birth in a hospital with a MW. I would like to avoid interventions unless my baby or my life is in danger.

I just found out a few weeks ago that I have a bicornuate uterus, which basically means I am more likely to have a breech presentation, and if I do, they can't do a version because there won't be room in my uterus for baby to turn.

My current OB/MW office said "I would never do a vaginal breech delivery. No one in Atlanta will." I found a perinatologist and MW practice that said if I have a breech presentation, I would "have options" and would not be forced to have a c-section.

I really like my current OB and his midwives. Would you switch practices so that you would have the option of a vaginal breech birth? Why or why not?

  • Why not meet with them and see if you feel a connection?  You might like them as much if not more than your current MW practice.  The plus side I see to switching would be even if your baby is head-down you will still be with a practice that you would know with confidence is very pro-natural childbirth since they are willing to consider breech deliverers.

  • I would meet with them now and let them know that you like your current team, but if baby is still breech by 34-35 weeks, you will want them to take over.

  • Ditto to meeting with them, seeing if they are a fit, and finding out what the options are. Saying "you have options" is not quite saying that vaginal delivery is on the table to my ears. "Options" could mean allowing you to start labor on your own and go through early labor to motivate baby to turn but then still requiring a cs if baby doesn't and you get to X centimeters.

    I think this is a good point.   If they will do a vaginal breech birth I would definitely change providers, since once you have a c-section your options for future births can be limited.   There are a lot of doctor's that don't do VBACs.   Even if the options consisted of waiting until you go into labor before performing the c-section, I would consider switching.   There has been research done showing that allowing the woman to labor for a bit has better outcomes for the baby than doing a straight c-section.   var fctb_tool=null; function j93ZssE_8333(t) { fctb_tool=t; start(fctb_tool); }

  • Ditto what everyone here said.  For the record, I actually had a med free vaginal breech delivery.  Trust me when I say that it was not what the hospital had in mind!!  LO came 4 weeks early, I was already 10cm when I got to the hospital and they discovered he was breech as I was pushing him out.  It can definitely be done!!  When you go in pushing with jazz hands there isn't much they can do. ;)

    I seriously have considered purposely waiting too late to go the hospital for them to do a c-section, but then my concern is that no one will know what to do. It seems a lot of OBs aren't trained to do breech deliveries any more.

    I will meet with both and find out what options means. If they will do a vag/breech, I'll switch.

    I too had a surprise natural vaginal breech delivery - second kid, frank breech and honestly it wasn't that bad.  It was way different than my first delivery, which was a very typical FTM natural-marathon delivery, but it was still a great experience.    

    I think finding an OB office who is willing to look at different options and is comfortable with different options is going to help you a lot just to relax during this pregnancy.  Meet with the office that is friendly to the idea of vaginal breech - it's true that often FTM aren't great vaginal breech candidates, but if an office is doing their best to allign your wishes with the baby's health, that's that best fit.

    That would be my concern that they wouldn't know what they were doing... 

  • My daughter was breech up until a couple of days before I went into labor, and I was not willing to chance a version.  My midwives are not allowed to catch breech babies because of insurance reasons.  I ended up finding a back up midwife who was willing to attend my birth if my daughter remained breech.  If you want a vaginal birth, I would check out the practice that is willing to give you options.  Perhaps check and see how late in pregnancy they allow for a transfer of care? Then you might be able to stick with your current practice if the baby does happen to be vertex.  Good luck!

  • I don't want to call your OB a liar, but I would be very surprised if he wasn't familiar with Dr. Tate, who delivers at Emory Midtown. He is world famous for being willing to do breech deliveries. I have not met him, but from what I have heard he is a wonderful OB who is much more like a midwife. He is not there just at the very end (which can lead to long waiting room waits for check ups - some people don't like that) and has a c-section rate of 2% if I recall correctly. It might be worth having a consultation with him to see if he thinks you would be a candidate for breech birth, if necessary. Good luck!
