Over Kill?

Now, I know that us mothers will do ANYTHING needed for the wellbeing of our children and I will do so if necessary, but part of me thinks that my doctor is just going overboard.

 

Background: During my first pregnancy I had active kidney disease and GD due to the kidney disease. Since then, my blood sugar is back in the normal range, my blood pressure is fine and I've been discharged by my kidney doctor as "inactive" and it should NEVER come back as long as I don't touch a single NSAID.

 

Now, my second pregnancy is here and I'm only 9 weeks along at this point and everything is good so far, but my doctor wants me to:

 

1. See a high risk OB for a consultation due to my previous pregnancy

2. Have a First Trimester Screening for related, but unknown to me reasons.

3. Take THREE glucose tests--one at 8 weeks, one at 18 weeks and one at 28 weeks (this is, of course, if I pass the first two or if I fail, but pass the 3 hour)

4. Visit my kidney doctor despite being discharged and him saying getting pregnant will not do anything to my kidneys

5. Take a 24 hour urinalysis at least 4 times throughout the pregnancy (If you've never done one, you have NO idea how annoying this is).

 

Obviously, I will do it all if necessary for my and my baby's health, but I just kind of feel that it's over kill.

 

Is it normal to take 3 glucose tests throughout the pregnancy? Or is it usually "pass" and you're done for good?

  • Personally I be happy if my doctor was being proactive and just wants to make sure that they cover all their bases. I have several friends who are high their second pregnancy and they did three glucose test at least for a baseline.

  • I would also rather be on the careful side. I would definitely consult with the High Risk OB. I would probably not go to the kidney doctor if he specifically said that pregnancy won't change anything.

     

    The glucose test ... I think it can't hurt to do 2 or 3 to be on the safe side. 

     

    No idea about the urine thing but I would think that based on your history it might make sense to at least do one or two.. maybe not 4

  • I'd get the tests that were receommended; because I'm always happy to be over informed.  I'd also ask a ton of questions about why (s)he was recommending them - for the very same reason.

  • Well,

     

    My OB recommended the three glucose tests--I was just wondering if that was normal for patients who had GD in a prior pregnancy or if there was another reason.

     

    The high risk OB recommended the other stuff.

     

    My only issue with it is that my nephrologist (kidney doctor) is over an hour away and right now my time is VERY limited.

     

    We just found out my daughter may have myoclonic seizures, a type of epilepsy, and she's going to need an EEG and all these other types of tests to figure out which type of myoclonic epilepsy she has.

     

    She is being seen by a pediatric specialist in Johns Hopkins and so I have to take the whole day off of work to make these appointments for her, which we are spending 90% of the day in Baltimore (I live in Carroll and work in Howard).

     

    I mean, I will most likely do all the tests they want me to, I'm not looking for someone to tell me it's over kill so that I don't have to do them, I'm just wondering if this kind of thing is normal.

     

    Even with my first pregnancy, in which I was dealing with all these issues, they didn't have me do the glucose test until 24 weeks and they didn't have me do all these urinalysis tests either.

  • can you find a closer nephrologist?

    I doesn't really sound like overkill to me, its a pregnancy, you're high risk - so that means you need closer monitoring, which actually just means more testing.

  • I looked around me, but none really in my area. The closest one I had started at was at Mercy hospital and she didn't do a good job.

     

    I ended up with Dr. Rossiter because I was recommended to him by his wife, the high risk OB at St. Joe's.

     

    They haven't said I'm high risk, just that they wanted me to have a consultation with a high risk OB due to my past. So as far as I've been told, everything looks normal and no reason for me to be high risk this time around.

     

    The only reason I was high risk before was because I had active kidney disease and that shouldn't be an issue ever again, so I shouldn't be high risk this time.

    Has your OB been in touch with the kidney specialist directly?  Maybe rather than having her make recommendations in a black box she could speak to him/her and the two of them could figure out together what makes sense.

  • Weighing in on the GD aspect of the pregnancy - I had GD with my first and was told that when I had a second, I would need to do the following:

    - start following the diet the minute I found out I was pregnant (even better was to follow the diet while trying to get pregnant)

    - get tested right after finding out I was pregnant (glucose test)

    - get tested again at 24 weeks (I think this is pending the results of the 1st test)
