I replace everything for my pump each time - tubing, shields connectors etc. I clean all bottles and parts.I buy new bottle nipples too so they have to be boiled.
I tossed all of my bottles as they were plastic and very worn. I have heard friends recommend that you replace the nipple as after 2 years they can begin to degrade. As for my pump, I had to get a new battery (expensive) since mine would not charge anymore (Medela Freestyle) and I bought new tubing, breastshields and the phylanges (sp?). Basically anything that could have been in contact with the milk or is plastic and could degrade over time and depending on how much you use them the first time around, they could be pretty worn.

New bottle nipples (but not bottles) and new membranes (because those should be replaced often anyway, apparently- I never knew!). That's it. I'm tossing the steam/sanitizing bag we used a few times (they can each be used something like 25 times) but I have several more if Ineed them... we didn't use them often and generally only for tubing/pump parts.

I haven't washed bottles yet, because I do not plan on using them until LO is at least 4-6 weeks old. But I am just going to run them threw the sanitize cycle on our dishwasher and use them.
As far as my pump I have a Lansinoh Affinity pump. It is a closed system so I am just going to boil all the parts (tubes and membranes ect) and reuse the parts.
We'll get new nipples but I'm waiting to make sure #2 will take the Dr. Browns before I get them. I met w/ the LC at the hospital & she checked my pump parts (and suction) and said I didn't need to replace anything, just wash & sanitize.
I got a new pump and will boil the parts to it. DD's bottles weren't BPA free (before that was a known issue), so I got new bottles, as well. I will boil all bottles and pacifiers, as well. With DD, I periodically boiled all of that stuff since I didn't buy a sterilizer, so even if you are reusing, I would definitely boil everything before using.

BUT, the only reason I did this was because there was 5 years between the two, all of these items had been stored in a hot attic for at least part of the time, and they came out with BPA-free during this timeframe.
Between #2 and #3 (only 18 months this time), I will replace NOTHING. There is no reason to. I will just put all of those same items in the dishwasher on it's sanitize setting.

Dishwasher should sanitize just fine (ours does a great job - we have a special bottle parts basket that we bought for the top rack; works great).
