TTC after 35

Should I expect a Pap/Internal exam at my 1st appt with RE?

Just wondering if I should bring my hospital gown with me (yes I have my own!) because I HATE wearing the paper ones that rip and make me feel like I am trying to cover myself with a napkin.

  • Your first appointment will probably be talking only, so you will likely not be getting nakey.  Future appointments will include a vaginal u/s, which you just get undressed from your waist down and cover with a paper "blanket." 

    I used to hate going to the GYN for my annual PAP, but after these past 9 months of showing my vajayjay to several people several days/week for monitoring it's going to be a breeeeeeeeze!

  • Well... my first RE appointment was a sit down discussion.  She reviewed all my previous history, records from my OBGYN, family history, etc.

    Vitals : Height , Weight, temp, BP, pulse

    She ordered a lot of blood tests for various cycle days and instructed me to schedule appointments on the appropriate cycle day (CD 3 and CD 21) for blood draws

    She did a vaginal ultrasound to check my ovaries, uterus for any obvious abnormalties..

    Then I scheduled a follow up appointment when all the blood work was complete to discuss "next steps".  When I went to the follow up appointment we discussed having an HSG, and my various options for treatment : Clomid w/TI, IUI, IUI with Injectables, IVF.

    Here I am 4 years later working on IVF #2.

    Me = 39 DH = 50 TTC (since 01/2009 after a short break, I stopped counting *cycles* in 2010)
    DX 2010 : Unknown Infertility , AMA
    HSG 2005 = Normal / HSG 2010 = Normal
    SA 2005 = Normal / SA 2010 /2012 = Slightly low count : motility, morphology GREAT!
    Chromosomal analysis, MTHFR, ANA's, Lupus, Protein C, Protein S, APCR = Normal 2011

    M/C : 2005 (7wk), 2010 (6wk), 2011 (7wk)

    8 IUI's 2010-2011 all BFN

    IVF/ICSI antagonist 01/2012 #1 = BFN

    IVF/ICSI antagonist long 06/2012 #2 = BFN

    IVF/ICSI/CGH MDLF New Clinic/New RE 11/2012 #3 = One beautiful boy blasty frozen PGS normal

    IVF/ICSI/CGH MDLF 02/2013 #4 = One more beautiful boy blasty frozen PGS normal

    FET 6/20 : One 5AA PGS normal transferred Beta 1 6/28 = 53.9 / Beta 2 7/1 = 220 / Beta 3 7/3 = 550!

    1 U/S  6w3d - Heartbeat 119 /  2 U/S - 7w5d- Heartbeat 169 / 3 U/S (OB) 8w5d - Heartbeat still great!

  • Andplusalso, I meant to add that my RE did not do my PAP.  I had a current one through my GYN, so I just gave my RE a copy of the results. 

    I'm not sure if it had been expired if she would've done one right there or sent me back to my GYN for one.  (probably just done one right there)

  • I would say every doctor is different. I just scheduled my first RE appointment for June, and the lady I spoke with said to be prepared for a pelvic exam. Whether or not that means there will definitely be one, I do not know. Maybe you can call and ask?

    ETA: Now that I think about it, she may have added, "especially if you haven't had a PAP in a while." I have my annual appointment with my OB/GYN next week. So, maybe the RE and I will just talk?

  • I had my vaginal u/s during my first appt. and also had the sit down w/ the RE. Also ordered the 3 day blood work. I think we also discussed doing a repeat SA doe my DH since there was problem with the first. Good luck with your upcoming appt. Keep us posted.

  • My first visit with my Kaiser RE did involve an ultrasound. She did an AFC and then we sat there while I was on the table in that robe and talked through everything. 

    Hang in there, and write down all your questions - I always forget! 

    ttc since 2/2010 ~
    me (36): Hypothyroid (on Levothroid), low vit. d, borderline/high fsh (day 3: between 7-10) (day 10: 13 during CCCT), AFC: 14
    dh (31): awesome (minus one sample with agglutination)
    Diagnosis:possible DOR and/or unexplained + elevated NK cells + MTHFR (C677T - one copy)

    MAY 2011 - FEB 2012 - 3 injectable IUI's with numerous cancellations due to high TSH levels
    MAY 2012 - onto IVF/ICSI (Antagonist Protocol) on BCP and Folgard (3 week delay - cyst - boooo) 5/21 start stims 5/30 ER 11R 8M 3F 6/2 3DT of 3 6/12 Beta #1 83 | 6/14 Beta #2 196 | 6/21 Beta #3 3818 | 6/28 Beta #4 22,213 | 7/2 1st U/S - 2 on board! 8/24 CVS reveals that we have a boy AND a girl on board!

  • Thanks for all the info!  It sounds like I should be prepared for anything.  :)  I am SO looking forward to this!  I sent an email to my OB/GYN Nurse who has become a friend and she said she was glad I made the appt but told me to expect that they will blame my age.  I can deal with that.  40 is the new 30, right?  :)

  • Good luck girl! My appt was an hour worth of talking and planning, then two days later (since it was cd3) back in for the AF count and ultrasound...

  • Good luck Llama! My first appt with RE was just talking about what needed to get done like SA and the start of all testing that needed to be done for me. The minute she made a comment about my age I started crying! Big baby..that's me :)

    ME:46 MH:44 DE IVF 2014
    Met with RE 4/11. 2 IUI's BFN. DE best option. Switched clinics to do "shared" program. Had to retake all tests and a mamm that put me behind and then on a DE waiting list for 12 months. Picked a donor!! (10/13/13) Got matched. Estimated transfer in December. After 2.5 years of patiently waiting I will finally cycle....can hardly believe it. DE cycle got cancelled. One of her tests came back positive.  Waiting for another donor. Donor picked!! (1/18/14)

    DE IVF #1 (4/26) BFN  DE FET #1 (6/4) BFP! Beta 1=339 Beta 2=852 Beta 3=9957 EDD 2/22/15!!

  • Our first meeting was all talking - an in depth discussion based on the questionnaires and scheduling of testing. 

    The internal ultrasound (to look for AFC and cysts and a general look) needs to be done CD2-3 so if your appt is on one of those days, you might be able to fit that in.

    My first visit with my RE, we met in his office first and then int the exam room for a vaginal ultrasound. He did not do a PAP. He mentioned I needed one but had me get one from my regular doc. 
    Single Mother by Choice. Life didn't work out the way I planned so I did it on my own. IUI #s 1-3, unmedicated = BFN, IUI #s 4-6, 50mg Clomid, Ovidrel = BFN IVF #1: 23R, 20M, 17F. 5 day transfer 2 blasts. 2 Snowbabies BFP 6dp5dt, Beta #1 7dp5dt = 58, Beta #2 9dp5dt = 114, Beta #3 10dp5dt = 187 1st Ultrasound = 5/3, not much to see yet. 2nd Ultrasound = 5/17, TWINS!!!     My 1st appointment with the RE they took vitals, height, weight, bp.  We talked about history and went over blood work and an ultrasound that my ob did.

    She went over a plan and scheduled blood work, she also did a transvaginal ultrasound to look at my ute and ovaries.
