Not sure what to do

I'm on my first cycle of clomid and was told to come in on CD21 for bloodwork to see if I ovulated. I just got a +OPK yesterday (CD 18) and then again today. I called to see if I should still go in on Wednesday and the nurse told me yes. Wouldn't it still show low levels if I ovulated just the day before having bloodwork? I thought you were supposed to get bloodwork done at 7dpo?

    Yes, you are right on, you will not get the right result if you go in on CD21 is you got a + OPK on CD18.  You should go on CD27 (24-36 hours after positive OPK is O date).  However, my experience with OB's is that they still want that day 21 bloodwork, despite what you tell them the first time.  My RE doesn't bother with CD21 bloodwork, but then again, he doesn't deal with Clomid either.  Taking the Clomid will likely put you in the right range for CD21 bloodwork to be more accurate.

  • I got my Clomid from my OBGYN practice when we were trying for our son who is now 2.  On my first cycle taking it (50mg CD3-7) I didn't get a + opk until CD18.  We were out of down on 7 DPO so I went in before we left at 5 DPO.  I got the call while we were out of town that the numbers confirmed that I did O.  Come to find out we also conceived that cycle too!

    You are right, it is 7 DPO.  I don't know what that person on the phone was talking about.  However, if circumstances prevent you from going on that day, oh well, do waht you can!

  • You're right that your progesterone will probably be on the lower side if you go then. My RE's office always insists I come in when they tell me to also-they haven't cared when I was actually ovulating because they would say I "should have" ovulated when they told me I would, based on ultrasound/bloodwork.

    For me, I wasn't getting the positive OPK until 4-5 days after they told me I would so I was indeed ovulating later. Whenever I'd call to ask if I should move the bloodwork they'd tell me to keep it where it was. The progesterone levels were ok when they'd take it, but it was just showing that I ovulated, not that it was a good egg.  For this month I insisted on the Ovidrel injection to ensure that I was not releasing the egg too late.

    Did you have bloodwork and ultrasound earlier in your cycle? Did they say then that you would ovulate around that time or no? Some people do ovulate later and it's no big deal because the follies and lining are in line with that, it just wasn't the case for me.

  • I am just seeing my OB. I don't really understand why they would still want me to come in on CD21 knowing I just ovulated. I didn't do any other bloodwork this cycle. 
  • It seems like a lot of OB's go on the premise of "women ovulate on day 14".

    Since they didn't do any other monitoring, if it were me, I'd call and tell them I strongly prefer to push it back a few days because you know that you did not ovulate on Day 14.
