I did with my Playtex bottles (they are pretty slow anyways), but I didn't with our Gerber bottles. I wont be buying any either, since we are in the middle of switching to all sippy cups.
I'm trying to remember. Maybe 4-5m? However, he's bottle fed entirely as I EP. Since he wasn't getting it from the source, the nipple flow didn't matter as much. I just upped it when he was repeatedly flipping the nipples inside out (we use mostly Medela ones) and was taking absolutely forever to eat. It was taking us 20-30 minutes at every feeding with the slow flow ones.

I think we started fast flow nipples around 7 months. She was taking forever to finish her bottle and she wouldn't eat enough because it took so long, so we switched over to fast flow and now she finishes a bottle in about 5 minutes.
We will be ditching the bottle some time this month. Hopefully before she turns 1 and starts on milk.
I switched to fast flow at about 5 months when DD was collapsing the nipples and taking forever to drink a bottle. She started doing that again about a month ago, but there was no bigger sizes so I just poked a big hole in the middle with a scissor and it seemed to help for a while. Now she is inverting them again, so I have no idea what to do next. I've tried putting her formula in the sippy, but she only likes water out of a sippy. Go figure.
