Here's my plan (sorry of it's large, I don't know how to put something copied from Word into smaller font):
Birth Preferences
We want to thank you for being here to help us through this exciting time. While we are aware it is impossible to plan something as unpredictable as a birth, so long as things are going well, there are some things we would like to happen. We have discussed this with our OB. Obviously, we are aware this is a ?best case scenario? and might have to change based on circumstances and we trust your expertise. Thank you again for all your hard work!
During labor
?I would like to labor without pain medication. I am aware of the options and will ask if I change my mind.
?I would like a heparin lock only.
?Unless there is a risk of complications, I would like to eat and drink as I feel the need. My doctor has approved this.
?I would like to be able to move around freely, with only intermittent monitoring as needed.
?Because I will be laboring without pain medication, I would like to use a tub room and will be using the Hypnobabies method. Please keep the room dim and quiet when possible.
During birth
?I would like to push in whatever position is most comfortable for me
?So long as things are going well, I would like to use self-directed pushing
?I prefer to tear instead of having an episiotomy.
After labor
?I would like the baby placed on my chest immediately following birth. Any tests or cleaning can be done during skin-to-skin time or afterward.
?I would like to breastfeed immediately following birth.
?Please do not separate us from the baby unless it is medically necessary and, barring an emergency, has been discussed with us first.
Here's mine. It was one page with the part about the need for a c/s on a seperate page. I must say that the hosptial followed my plan really well.
Birth requests for theladyjess
We have decided on a med-free natural childbirth. After much thought and research, and barring any unforeseen complications*, we ask that you honor the following requests:
Labor:
?To be allowed to labor for as long as it takes without being pressured for time.
?To have freedom of movement and labor in any position I find comfortable.
?To eat and drink freely.
?Please don?t offer pain meds.If I need them I?ll ask for them.
?Please keep disruptions limited.
?Limited cervical checks.
?Please only use fetal monitoring intermittently.
Delivery:
?To be allowed to push and/or deliver in any position I find comfortable.
?To push at my discretion.
?To not have any time limits on how long I can push.
?I?d rather risk a tear than have an episiotomy. Please use perineal massage/support, warm compresses, etc to reduce the chance of a tear.If an episiotomy becomes necessary I?d prefer to have a pressure episiotomy.
?If I have a tear or an episiotomy please use a local anesthetic during the repair.
?If possible I?d like my husband to have the option of catching the baby if he desires.
?Please place the baby on my abdomen/chest.
?Please wait until the cord stops pulsating before clamping/cutting.
After delivery:
?I?d like to deliver the placenta spontaneously and without pitocin.
?I?d like to breastfeed immediately.
?Please examine the baby in my arms.
?I?d like to get up and walk as soon as I feel up to it.
?Please offer us the chance to give our baby the first bath.
?Please allow unrestricted access of the baby to Jim and Nancy Bennett.
DO NOT give our baby the following:
?Hep B vaccine. We will start the series at the two month check up.
?Eye drops/ointment.
?Formula, sugar water, pacifiers or artificial nipples.
*Should any unforeseen complications arise we ask that we be given time to discuss our options and come to a decision privately.
In the event of a Cesarean Section:
?I?d like to be with my husband at all times. Please don?t separate us.
?Please explain the surgery as it happens.
?Please lower the screen so that I can see the baby as it?s delivered.
?Please leave one arm free so that I can touch my baby.
?I?d like my husband and baby to stay in the OR with me for the remainder of the procedure and accompany me to recovery. If this is not possible I?d like my husband to go with the baby and my mom to meet me in recovery.
?I?d like my incision to be closed using sutures. Please do not use staples.
?Please don?t allow anyone other than my husband to hold the baby until I?ve had a chance to in the recovery room.
?I?d like to breastfeed as soon as possible.
In the event of our baby needing to be transported to the NICU we request that my husband or mother accompany the baby and that I be discharged as soon as possible to be with my baby.
