I posted full story on the Vbac board . Just wondering if any of you had an transverse cut with an extension cut that went up ??? And if so have/will your OB let you try for a vaginal birth???
I had a horizontal incision with a vertical extension cut. The only reason I even know this is becasue I got a massive abcsess and infection that they believe was from the vertical incision opening a little, as my uterus shrank down and the stitches didn't hold. I don't know how big the extension was but they had to do it because DD was wedged in and they couldn't get her out fast enough. I had my emer c/s becasue DDs heart rate was too high and she wasn't tolerating a vaginal delivery.
At my 6w PP check up, my OB told me it would be my decision if I wanted to attempt VBAC or have RCS. We didn't go into it any further becasue I'm not planning to have another one for another year or so. Again I don't know how big the extension was, but it was big enough to need a few stitches, and it was vertical, and I was told I could attempt VBAC if I wanted to.
BFP #1 August 2007, Lost Nov 2007, no heartbeat found at anatomy ultrasound at 19 wks
BFP #2 March 2011, Baby Girl born November 2011!!!
I don't know the terms, but I have a vertical cut on the inside and a horizontal cut on the outside. I am not a candidate for a VBAC because of the internal cut not being strong enough to handle contractions. I've since had a RCS and I'm really not a VBAC candidate because I now have a horizontal and vertical incisions on my ute.
I ended up with a lateral extension from my transverse cut during my emergency c-section. My DS went into fetal distress during pushing and they called an immediate section to get him out after forceps failed.
She said because my uterus had been through so much trauma from laboring and he was so far down into the birth canal - my uterus ripped while she made the incision. I remember asking the dr if this meant I'd have to have sections from now on while they were stitching me up and she said yes.
Now that I'm pregnant again, it was one of the first things I investigated. I've had several OBs from different practices (even my own who is very pro vbac) and they all said the risk is too high and they'd recommend a section.
I'm not sure if that risk correlates with the type of extension. I just don't think there is enough data for it not to be a considerable risk.

I am getting my OP report from my Old OB. I want to know what it say's.
My New OB said my OP report was hard to understand. She also said having
that extension cut and not knowing what way it goes either strait up or
how ever, she is not in favor of a Vbac. I called my old OB and she said
everything is in my OP report and states everything clearly.... So IDK
what's going on. I don't wanna go through another C/S. I have a 18 month
old to tend to... I have been very depressed and not my happy self
when found all this out last week wed.

The reason I had the extension cut was because my old OB cut into the 1 fibroid I had as she was cutting into me. I had the lower transverse "bikini cut". From what my new OB say's it's not in my OP report what way or how long it went and the OP report was not very detailed. But when I called my old OB office she said everything is in my OP report. So IDK what to think. P.S According to my old Ob office it say's in my OP report my extension goes strait up. The fibroid I had was on the side down by the cervix and my old OB had to make the cut upward. So IDK what that means... I am getting my OP report.
I wanted to clarify that my extension was a tear that formed from the incision the doctor made while they were pulling the baby out. The cut wasn't a clean transverse line, more like an oddly shaped T.

In any case, it sounds like the extension created a non-simple incision site. There isn't much info out there on "extensions and vbacs" unfortunately.
Try and have a face to face with your old OB with your operative report. It was a tough pill to swallow for me but I'm OK with my upcoming scheduled section. Good luck!

Yeah, I tried looking for info on it and I got nothing. What would you do if you where me??? Would it scare you not to have a Vbac ??? IDK what to think .

For me and my family, I chose a RCS with this baby. Once I exhausted my desire for a vbac, I made peace with my decision and will try to make the best of this experience. I'm at a teaching hospital and while they said I could Vbac if I wanted, they recommended a RCS. I had a meeting with my personal OB and one of the most seasoned OBs at this practice and they said while the risk was small (they don't have data on vbacs/extensions/ruptures specifically) it was big enough for them to recommend a RCS. I'll mention again I'm at a very vbac friendly practice and hospital.
Meet with a few other OBs to find your closure. The hardest part for me was figuring out which direction to choose. Once that was clear, I felt so much better.
