I am from a blended family and have a not so great relationship with my step-mother. I am pregnant with my first child. My father is really excited and while I love him I can't stand the thought of my baby calling his wife "grandma". So my question is what should my child call my step-mother? Both my mother and DH's mother are both fully in the picture too, they will be "grandma".

That's got to be a personal decision. You could do just her first name? Or "Nana FirstName"?
What did you call your stepmom?
You can see my Easter vent below. DH has many reasons to have an awful relationship with his stepmom, but he's put it all in the past to focus on what's best for the boys. They call his SM "Grandma" and she's really been a great one to them, even though her position in the family is less than perfect.
theres plenty of others things you can ask the baby to call your step mother, but she needs to be encouraging of it as well.

Honestly the child could just call her by her name. MY nieces and nephews call their grandfather's wife by her fist name and they are all fine with it. (and there isn't even a bio-grandmother on that side) My brother-in-law just didn't have a close relationship with her and it felt wrong to him. The kids like her well enough and the name hasn't mattered to anyone.
EDIT Have them use her first name. I wouldn't push the grandma/nanny thing if you don't feel comfortable with it.
DS (and SS) have MawMaw (firstname), Nana, Grandma, Grandma (lastname), MawMaw (lastname), Nana (firstname) and GreatMawMaw.
And, SS has a few others on his mom's side. It's all personal preference and honestly, the kids will pretty much decide on their own. The above Nana was YaYa at first, then Nanny, Manna and now Nana.
That is what I was thinking.
