Slow rising hcg levels they think eptopic

I have slow rising levels to I found out I was pregant at about 3 weeks. I experencd a little bleeding I thought it was my period. I went to the E.R and I got all the tests don't they said I'm about 3 weeks on
April 22 hcg levels 527
April 4 hcg levels 535
April 5 hcg levels 737
April 10 hcg levels 971
I'm about 4weeks today that want mt to take methotrexate sodium Inj they think it's eptopic. I'm so confused they said they saw something near my overy and tube but they weren't sure what it was I'm so confused I have no pain I don't no what I should do.

Re: Slow rising hcg levels they think eptopic

  • I would get a second opinion first! I've heard of women who think they're having ectopic/miscarriage and it turns out they weren't and had healthy babies. Don't give up hope yet, but just get a second opinion to confirm. Goodluck!

  • I had a similar situation in November/December.  My hcg levels were all over the place.  Up a little, then down, then up again.  At about 6w, they did an u/s to see if they could see the ectopic.  It wasn't big enough so they couldn't see anything.  No baby in my uterus and nothing on my tubes.  At that point, they should have seen something in my uterus (at 6w) if it was a health pregnancy.  We did the methotrexate that day.  Unfortunately, my ectopic was too far along at that point and the methotrexate did not work and I ended up having surgery Christmas Eve.

I agree on getting a second opionion.  But not to freak you out, don't wait to long.  It's my understanding that methotrexate only works up to a certain point.  Once you get too far along you may need mutiple injections (which you dont want) or surgery.

  • I am so sorry that you are going through this. I wish that I could be more encouraging, but your hcg levels are consistent with an ectopic pregnancy :-(. At the least, they are not consistent with a normally progressing pregnancy. Most betas will rise by at least 66% every 48 hours in early pregnancy, and failure to rise 50% is confirmation of a nonviable pregnancy (regardless of location). Because of very rare cases (like vanishing twin), it is common practice to to continue to monitor hcg levels until they reach the "discriminatory threshold" which is about 1000-1500. At that point, you should definitively be able to see an intrauterine pregnancy if there is one by transvaginal ultrasound.

It is frustrating, but oftentimes, the ultrasound findings are not conclusive in ectopic cases (in terms of actually finding the pregnancy outside of the uterus).

PP mentioned that ectopic pregnancies often do not present (show symptoms) until about 6 weeks on, and that you will definitely feel it. However, it is vital to understand that a large number of ectopics are actually asymptomatic until rupture (or at least until they cannot be medically resolved, and thus require surgery because they are too large).

PP mentioned methotrexate only works to a certain point and this is true. It is most successful under 3500 hcg and contraindicated over 5000 hcg. Also it is not an option if a heartbeat is detected or if there are symptoms of rupture.

I hate that you have to go through this, and these are not easy decisions to make. I hope that it resolves soon, and that you find peace. I know it is hard. I have been through it myself. I will be thinking of you, and I hope you report back and tell us how you are doing.

  • I am in the E.R. As I write this :/  I am coming in foe a second opinion my dance and I are hoping for better hcg today's . I just had my ultrasound done and transvagenal agin I'm nervous. I hope it's just a cyst. I have no symptoms besides lower back tightness and crams like I have gas bubbles. When they were doing my ultrasound it kinda hurt on my right side :( not good considering that's the side they saw the mass. Mumom and grandma have slow rising number to my mom had higher numbers when she had her eptopic then when she was pregnate with my brother. Her eptopic ruptured though. I am so early my mom said hers did not start to be normal tol about 6 weeks

  • I've had two ectopic pregnancies (one on each side). With both I went into the ER around 6 weeks with heavy bleeding. I thought I was having a miscarriage. With the first they couldn't decide if the "mass" was in my left tube or not and because I wasn't in pain I was given the option to wait 48 hours to check my HCG levels again. They even let me go home to wait it out.... I woke up one day later in terrible pain. Rushed back to the hospital and given the injections. A week later I was supposed to get another injection because my levels weren't dropping fast enough. The lab messed up and did an HCG test instead of my liver test, which saved me from more injections. About 6 months later I had an ectopic on the other side. Same bleeding symptoms. This time I was admitted to the hospital and the next day they gave me the injection because my levels were dropping fast. Turns out the ectopic's damaged both my tubes and I ended up having one removed in December. But that's beside the point... I would have them keep checking your HCG levels and doing ultrasounds. But be sensible, you don't want a rupture!
  • Oh, and you DON"T ALWAYS FEEL IT.... just so you know.
  • Thank you everyone so yesterday I was in the e.r. Again I got my

Levels checked again and ultrasouns. I'm still to early to see anything I'm only 4 weeks. They said the mass they saw hasn't got any bigger and hasn't moved so that's good. I was really dehydrated yesterday and had a headache so they hooked me up to a I've for fluids. My hcg levels came back lower then my last one. It was 971 down to 951.  I have hope threw my family history I'm not ready to give yet I got a new obgyn I'm meeting with tomorrow because the one I was seeing kept pressuring me into aborting it she didn't even care about my family history. I need a second opinion. I just hope by 5 or 6 weeks they see something in my uterus I'm only 21 and newly Engadged :) my fianc? is always by my side.
