We use Avent bottles and had no clue when to switch so went by the months on the packaging at first. But when it was time to move up to size 3 at 4 months, he wasn't ready. He was getting too much with each suck and choking. We waited to switch until he started to seem a bit frustrated and antsy while he was drinking.
You could always try the next size up and see what happens. In our experience, we knew pretty quickly if he was ready or not.
He basically told us he was ready b/c he would be really frustrated when he ate and that was unusual. We were on level 1 nipples (dr brown) for a long time.

If you plan on breastfeeding at all it is recommend you stay on slow flow indefinitely. If you are only bottle feeding I'm not sure what the recommendation is, but there is definitely no harm with staying with a slow flow nipple.
