If he doesn't have a scooter - Toys R Us had one on sale for $20. All the kids in our neighborhood ride them all the time (the 2 wheeled kind).
I recommend asking what he's into. The 5th birthday parties we've been to have varied widely in terms of interests. Some kids are crazy into Transformers, others are nuts for Star Wars.
One of the biggest hits from my 4 yr old's birthday that my 5 yr old played with non-stop was one of those huge cardboard things you build and then they get to color all over them. It was an eye sore but they spent hours in the one we got (off to find pics and link)
Target carries them in store too for $20 in their crafts section.
It's HUGE so the kids get really excited about opening it.
