We used California Cryobank and paid $465 a vial for an anonymous?donor. (open donors cost more). My RE only does Back to back IUI's so we needed two vials for each cycle.?
The cost of shipping the vials to our RE's office was around $200 each cycle, and then we paid OOP for the back to back IUI's which cost us $1600. There are a bunch of different banks and they all vary in what they offer and how much it costs. Good luck.?

we are using xytex. Our vials (iui prepared) are $565 for id disclosure. They are a little cheaper if they are not pre washed and if you don't do id disclosure. There are some much cheaper banks cali, xytex and fairfax seem to be the most expensive. NW Andrology and Midwest are are much more reasonable in price.

We are using Fairfax. ?We paid about $450 per vial (can't remember exact amount) for an anonymous donor and bought 8 vials because they were having a "buy 8 vials get 2 years of free storage" special. ?We have a local office, so we pay $45 to "withdraw" our vials but don't pay for shipping, since we pick it up ourselves. ?Our RE does b2b IUIs for frozen/donor sperm, and my ins pays 50%, so one cycle costs us about $450 for monitoring/blood work/IUI, but the sperm has already been paid for. ?Hope that helps! ?

Something else to check into... our RE recommends "ICI ready" samples vs. "IUI ready." ?IUI ready samples have been washed and then frozen, ICI samples have just been frozen. ?Because your RE will wash the sperm anyway, you'll have a better # if you only wash them once. ?Just check w/ your dr and see what he/she recommends.
