DD used to ball her blankets up in her crib for a pillow so we got her a toddler pillow from www.bobbleroos.com; now DS uses it; they were both about 18 mos old when they started; DD now has a youth pillow which is smaller than a standard but bigger than the toddler.
Gave her a pillow when we put her in her big girl bed, right at about 2 yrs.
~ Diagnosed with PCOS in March 2009. Taking Metfomin for insulin resistance and irregular cycles/ovulation.
Had healthy baby girl in Jan 2010 via C-Section (HTN and Breech Presentation). ~ Miscarry at 8 weeks with D&C in May 2011 (low progesterone). ~ Had a healthy baby girl in June 2012 via C-Section (on prometrium to sustain). ~ Surprise pregnancy at age 40! Baby boy due April 26, 2021 (took prometrium to sustain once pregnancy was confirmed, 3rd C-Section planned).
With DS we put a pillow in his crib at 1 year. With DD we did it at 6 months. They both slept 100 times better with a pillow. We have an angel care monitor so I wasn't worried about it. Actually for DD, we put the pillow under her crib sheet so she couldn't get under it. I know it's controversial, and we've been given the for doing so, but it worked well for us.
B's had some form of pillow (folded blanket, small pillow, actual pillow) since he was old enough for us to feel confident he could get away from it if it got on his face. He really likes pillows.
He currently has (at his request) two pillow pets, three mini pillows, and two full-sized (one is a king) pillows in his toddler bed. They usually wind up stacked three high for his head, and the rest get thrown to the end as sort of make-shift feet warmers for when he blankets get balled up.

DD had a pillow in her crib at 15 ish months. DS does not yet have one. No reason why not.
