Are you going to let LO eat cake on his/her birthday? DS doesn't eat sugar or salt, has never eaten a cookie or anything like that not even cherios and I'm wondering if there's an alternative to regular cake. Or should I not sweat it and just let him eat it that one time?

We have had a couple birthdays recently and I have given her tastes of cake, so we'll probably go with a traditional cake with a whipped icing (not the super sugary kind).
Whatever you do, I would suggest giving her a little bit of whatever you plan to have her eat at her birthday just to make sure she doesn't have any reaction. I can't stand seeing people post PSAs about how their kid got sick and had a reaction after having cake for the very first time on their birthday and how horrible it was. Just use common sense and don't introduce a brand new thing on a big day like that, sheesh!
I don't usually like babies with food all over their face, but I do look forward to see some pictures of these birthday babies with cake everywhere....

ITA...... Lydia has had tastes of cake in the last month or so. I won't deny her the traditional cake smash. I will let her play with the cake and have a few bites. I'm not going to let her pig out and eat an entire cupcake by any means. Just a few bites won't kill her.
DD will be getting some cake for her birthday and will also get some at her cake smash pictures. She has tried cake before thanks to DH, and she has tried all the allergy foods at this point so no worries there.
