I've already started packing and plan to make this a running total list...ya know...in case I go into labor and DH needs a map!
For Me: 3 changes of clothes, yoga pants, sweatshirt, cheapie underwear, Always infinity pads, hemorrhoid cream, stool softener, toiletries (lotion, shampoo, conditioner, body wash, iPad, iPhone, Camera, Video Camera, all chargers, batteries, sneakers, socks, flip flops, nursing bras, nursing tops, nursing pads, nursing cover, pillow, and snacks.
For Baby: vaseline (having a boy), lotion, rattle, swaddling blanket, regular blanket, 4 outfits, socks, onesies, diapers, wipes, Q-tips, diaper rash ointment, pacifier, hat for pictures (our hospital comes around to take professionals), bibs, burp cloth.
just FYI - my hospital didnt give barely anything to my sister except a pack of diapers which she ran out of, dry wipes you had to run under water, and some formula. Nothing else. So I wanted to bring some of my own stuff just in case. I'd find out what your hospital provides before you overpack like I plan on doing.
In terms of clothing I have pajamas, a nightgown/robe combo, an outfit to wear home, several pairs of comfy underwear, two nursing bras, slippers, and flip flops. DH has pj's and a couple changes of clothing.
For Baby I'm bringing a onesie, a romper with feet, a hat, and mittens. We were going to pack newborn and 3 month outfits, but we're going straight to 3 months now because she's measuring large and the starting point for those outfits is supposed to be 8 lbs (she was estimated at being 8 lbs two weeks ago).
Then I have panty liners, pads for my nursing bras, my breast pump, my birth plan and workbooks from Lamaze, a couple birthing books with some highlighted passages, pictures of me and DH, Baby's ultrasound pics to remind me of the light at the end of the tunnel, snacks for DH (and for me after LO is born), our baby book so we can have her footprints placed inside, a camera, and phone chargers.

This isn't necessarily true. My DD was 7 lb 8 oz when she was born and 19 1/2 in long and she didn't fit into 0-3 month clothes until she was at least 3 months old already. She gained weight just like she was supposed to and there were no issues. But it's good that you are trying to be prepared and bringing both newborn and 0-3 clothes. My sister was told her baby was measuring big also but he was only 6lbs 4 oz when he was born.
the right one will always make you feel like you are the most loved person in the world
