I can't remember what I packed the first time, and what I actually needed.
I'm also having a hard time knowing what outfit to pack for myself when I leave - it was summer when I had DD so wore a maxi dress. I'm not sure what to pack for when it's cold.

I haven't packed yet, but here's my list

hairbrush
charger for phone
camera
laptop
I wore black maternity yoga pants and a maternity t-shirt home both times.
I had a short hospital stay this time and didn't use much from my bag other than the camera, my iPad, my brush, my toothbrush, and my cell phone and iPad chargers. DH didn't even used the clothes and toiletries we packed for him because he ended up going home to spend the night with DS.
ETA: One other thing we used: snacks for DH in L&D. I think we had some trail mix and maybe some pretzels and a bottle of water for him.
Here's what I packed in mine recently:
Nursing jammies, tanks & bras
Yoga pants
Boxer shorts
Pajama pants
Pillow
Underwear
T-shirts
Robe
Socks (with grippy feet)
Flip flops
Breastpads
Toiletries bag (deodorant, toothbrush, tooth paste, face wash etc.)
Nipple cream (ie. Lanolin)
Phone charger
Baby's coming home outfit (with hat and gloves)
The only thing I didn't see on the list so far is hair ties/scrunchies or whatever you use to pull your hair off your face.
As far as what to leave the hospital in, I just wore black yoga/maternity type pants, a t-shirt, and crocs. If it has been winter, I would've just added a fleece and coat.

Other things to pack:
- paperwork for the hospital
- birth plan
- headband (if necessary)
- burp cloths (DD and DS2 were spitters from the moment they were born and the hospital didn't have anything I could use)
- an empty bag to carry home all of the freebies / random extras
- gum / mints
- chapstick (my lips were SO dry after having all three kids)
My going-home outfit in November was velour yoga pants, a maternity tank top, and a hoodie. I couldn't zip it up very well, but it was more forgiving toward my immediately postpartum body than a regular shirt, and it made nursing DS2 easier while we waited around forEVER to get discharged.
