I am using the infant insert. My daughter is 23 inches, 11 lbs. and she seems to love it. At 7 weeks, as soon as she's in it, within minutes she's knocked out. Even when it gets a little warm, she seems to stay asleep. She loves it. I wouldn't recommend using the carrier without it, simply bc our LOs don't have the best ability to support their own heads.
I would use it, but I have been using my moby more often lately.
I tried it the first couple of times, but didn't really like it. DS' feet just wouldn't stay curled up on my chest and I felt him sliding down. I'm sure it was more operator error than anything, but I hated having to adjust him and think he was squished.
Also I didn't feel like his head was supported enough.
All that said, I would still use it if I opted for using the ergo over my moby. DS is 10 lbs 8 ozs.
I use my Moby all the time but have used the Ergo too. I'd use the infant insert, absolutely. It's recommended for use until babies are able to support and control their head on their own.
