How many IUI's before IVF?

Hi,

 

Nearing the end of the second IUI'S and I am pretty sure I am heading toward a BFN.  I can feel the cramping.  So I was wondering how many IUI's you would do before moving on to IVF?  I'm sure each situation is different.   My dh has amorphic sperm but count is good.  I have no discernible issues.  Would you move on to IVF?  Or keep on the IUI'S?

I'm sure everyone is different but I'm just curious.

  • That's really your call. Is insurance paying for any of it? If it was, I would try a few more IUIs. If it isn't and you could swing it, I'd go to IVF to up the chances and hopefully save (some of) the aggravation. That's just me. GL!

  • It depends on a lot of things: your dx, your DH's dx, insurance coverage, etc.  I think that the general recommendation is to try a specific line of treatment no more than 3x, and if it doesn't work, then your chances with it are significantly lower.

    For us, we are oop for everything, I have severe endo, and we have slight MFI. After 1 natural IUI, 2 Clomid/IUI cycles, and 1 injects/B2B IUI cycle, we are moving on.  Our chances are very slim with IUI, the endo is likely coming back after my lap in August, and I'm not getting any younger : )

  • I'm in a situation now. I've had 2 failed natural IUI's and just had a consult yesterday to see what we can do next. I was not prepared for her suggestion of IVF. I have Endo and DH has no issues. I thought my RE would have suggested a medicated IUI cycle, but she explained to me that she believes it best for us to move on to IVF. DH and I were not really ready for that so we have a lot of thinking to do. But my RE did say she wanted to move forward after the 2 IUI's even before we started the IUI's. I would maybe suggest talking to your RE about this to see what she suggests. I can't say that we've decided anything yet on what we want to do because we haven't really been able to talk about it since DH was OOT until late last night/early this morning.

    GL to you in your decisions, it's not always an easy thing to do.  

  • Ditto PP's on it really being up to your individual situation.

    That being said, my RE "strongly encourages" moving onto IVF after 3 IUIs for the same reason PP mentioned - pretty much if IUI hasn't worked in the first three shots, your chances of success drop significantly.

  • Thanks for all the information.  I'm going to set up a meeting with our doctor and figure this out.  Our insurance has a lifetime max for infertility, so I will need to see where we are.  I know it isn't a ton of coverage and lord knows these treatments are pricey.  

     We have done two medicated IUI'S with no luck, maybe it is time to move on.

     

    Thanks for the advice, makes my head stop spinning for a little anyway. 

  • Good luck, It's hard when you are not expecting news the dr has.  I hope you find the right path that leads you to your BFP.  I think our doctor said two/three IUI'S before IVF, but I'll confirm that on Monday.  Now, if I can just stop feeling like s$&t and enjoy the weekend.  Oh what fun!
  • Age was our dr's main concern for me.  Since he wants to allow time to have a sibling later one, he didn't think that we should do too many IUIs.  (We are on our second now and will have a consult if this does not work).
  • What percentage is amorphous? That makes the biggest difference. If it's over 4-5% then you can get away with more IUIs than someone who's sample is 0%.

  • Our count/motility is okay. 25 million/mL, 41% motility prewash for IUI, but we only ended up with 2 million, 75% motility post wash. We also have extremely poor morphology, but they don't check it in the IUI sample.

    We are 100% OOP and we decided to move onto IVF if IUI #1 fails.

    If you posted more detail about the numbers, I could tell you what I would do in your situation. Good luck!
