DOR ladies-what protocol finally worked?

I have my WTF appt on Friday with my RE after our first failed IVF cycle.  I was on bcp for almost 2 weeks, then did the mirco dose lupron flare.

I produced 8 eggs, 5 fertilized and we transfered two.  I'm not sure if I just ended up on the wrong side of the odds, or if another protocol might be a better fit.

  • I'm DOR and I've only ever done antagoist with a natural cycle start (no BCP). Instead of BCPs, I take Ganirelix from 10dpo for the previous cycle and take it until I get AF, then start stims on Day 2.  

    For what it's worth, after 2 failed IVFs (post-loss) on Antagonist, I got a second opinion at another RE and he would have done microdose lupron flare with BCPs.  I went back to my RE with the second opinion guy's ideas and my RE said that I could do MDLF with the other guy but that he promised I'd be back to him in a month begging to him to treat me with antagonist.  My RE said that he spends his days getting patients who went to the other guy for MDLF pregnant using Antagonist.  I never went to the other guy and got pregnant on the 3rd attempt (post-loss) on antagonist. 

    My highest FSH was 23.3--scary high but here I am 27 weeks pg, after only getting 1 egg!  I love my RE!!!!!

  • Hey Manda, sorry to hear your IVF didn't work out.  

    The other protocol that seems to work for a lot of DOR ladies is antagonist.  That is what the RE was planning to try with me b/c of my low AFC, he was concerned that doing BCPs and even the micro-lupron might over-suppress my ovaries and reduce my chances.  But micro-flare is actually more successful for some DOR ladies, it's just hard to know what's going to work for you until you've tried it.

    It sounds like you got a decent number of eggs produced and fertilized (by DOR standards, at least), so I wouldn't be shocked if your RE suggests trying the same protocol again because it doesn't seem like it over-supressed in your case.  The way DOR was explained to me, it's just a matter of cycling over and over until you find one of the "good" eggs, if that makes any sense.  It sucks, but it is what it is. 

    I can't remember -- are you already doing DHEA, yoga, acupuncture, and all of that "extra" stuff that supposedly increases the odds?  If not, those might be worth talking to the RE about as well.  My RE was very skeptical about all of them, but I did all three anyway for the couple of cycles before my suprise bfp.  I honestly don't know if that's what helped in my case, or if it was just dumb luck. 

    Good luck.  I know it's hard to know what is the "right" thing to do because so much of it is trial and error.

  • For me it was an estrogen priming protocol.

    My first attempt at IVF they had me on the MDLF protocol, and I got cancelled...my progesterone was through the ROOF high, and they said the protocol didn't keep me suppressed as it should.

    With EPP everything went very well...and I got a TON of eggs (in my standards--18 retrieved, 15 were mature, 10 fertilized with ICSI).  We transferred two blasts and one stuck...and I even have 3 frozen!

    It took 44 cycles, just over 3 years, 6 failed IUI's in MI, and 1 round of IVF at CCRM to get our BFP!

    Beta #1 (9dp5dt) = 206, Beta #2 (11dp5dt) = 438
    1st u/s @ 6w5d = 11/11/11 = ONE little bean! HB 120bpm!

  • I have only ever done Antagonist because I am allergic to Lupron, but my RE said he would have done that with me anyway based on my prior cycles. All my blood work has come back normal for someone my age, but he still dx me with DOR because even on the max dose of stims I only got 11 eggs and only 2 were mature at ER. I did do a month of BCP before hand just to synch me up with their cycle. He says BCP has very little effect on suppression.

  • mine was 100mg clomid...2 mature follicles = twins.  cycle before that it was 50mg clomid = chemical pregnancy.  i dont really know what went wrong/what went right.  my stats are way worse than yours:  im 32, last july AMH was .22, FSH was 12 last may. 

    if i remember correctly you are still very young...im so sorry you have been having a hard time but you are one of the ones with DOR that i feel will definitely get pregnant eventually.  unfortunately with DOR patients sometimes it takes a much longer time.....i got really lucky.

    And Elf-I did accu with my last IVF and will continue in b/t cycles and with my next IVF.  I am back on DHEA as well-I figure if it can't hurt then its worth it!

  • I'm not sure if I can be of much help.  I got pregnant on Clomid/IUI with an AMH of .54.  I honestly think what got me pregnant was a whole lot of prayer and a miracle.  But I did want to wish you luck!  Also, there is a website (but I can't remember what it is) where women with DOR/POF post what worked for them.  A suprizing number have had alot of success with supplements either in conjucntion with ART or on its own.

    IF DX: DOR & Fragile X pre-mutation carrier
    2011: FSH 13.3 & E 99; AMH 0.54 2nd FSH 6.2 E 40's AFC: 8
    BFP from Clomid/IUI ~ Pre-e and IUGR during pregnancy ~ DS born 9/4/12
    Feb./March 2013: AMH less than 0.16 (undectable) and AFC = 4;
    BFP from supps ~ DS#2 due May 2014

    Actually, DHEA can hurt.  I don't know your specific history, but DHEA is only found to be helpful in women who have a certain hormone that is low.  I can't remember specifically what hormone it is but I think it is a certain type of testosterone.  At any rate, if that specific hormone is already high in you and you take DHEA, it can actually make it even more difficult to conceive. My RE told me in no uncertain terms to RUN away from DHEA.  He was more ambivalent about many of the others - wheat grass, CoQ10, B6, etc. - but made it clear that DHEA was a no-no. Good luck to you!

  • Long lupron with EPP before starting, ICSI, AH and a 2 day transfer while being treated for immune issues is what finally worked for me.

    The one cycle that I did antagnoist my egg quality was really poor.  I did a different variation of long lupron for IVF #1.

    ~SAIF/PAIF/Everyone Welcome~ 

    Me= 37 and DH = 41 

    Dx: DOR, Endo, APA+ (really high beta 2 glycoprotein antibody and high everything else tested), heterozygous MTHFR mutation, positive for lupus anticoagulant, high FSH, low AMH and both tubes blocked (per HSG on 3/8/11)

    IVF #1 - long lupron (with HGH, intralipids, lovenox and BA); 4 retrieved, 3 fertilized; ET 2 blasts and 1 frozen = BFN

    IVF #2 - a version of antagonist with EPP (with HGH, intralipids, lovenox and BA); 6 retrieved, 4 mature, 3 fertilized, 2 blasts and 1 frozen blast transferred on day 5 = BFN.

    IVF #3 April was postponed to May, May was canceled. June/July was canceled. Had a cyst aspiration and then began IVF #3 in August. ER on 8/22; ET on 8/24 with AH. +HPT on 9/5. Beta #1 (11dpo) = 3; Beta #2 (15dpo) = 29; Beta #3 (17dpo) = 60; Beta #4 (19 dpo) = 118. Heartbeat at 6 weeks 6 days =132.  Lil is here!

    TTC#2:  Trigger + TI = BFN; Clomid + Trigger + IUI = BFN.

    IVF #4:  BCP + MDLF + Lovenox = 7R, 1F = Transferred 1 6-cell embryo on day 3 = BFN

    IVF #5:  MDLF + Lovenox = 4R, 1F = Transferred 1 10-cell compacting embryo on day 3 = BFN

    IVF #6:  (New RE):  Long Antagonist November 2014 (transferred two 8 cell grade 1 embryos and froze one blast) = BFN

    FET#1:  BFN
