I just recently attended one. It's when it's not the woman's first child (i.e. it was her 2nd child/ 2nd pregnancy). A shower is for the woman's first (be it one or more babies). It was a very casual vs. her shower with her first son. For ex., most guests just got diapers and small gifts vs. big presents.
A sprinkle is usually for a second child. It's usually smaller, less $ gifts, more of a celebration of the baby than specifically to get the mom ready for the baby. Usually you give gifts like diapers and gender-specific items if the mom already has children of the opposite gender only.

I think Sprinkles are just very close friends and family. Just small gifts. We had one for my friend for her second baby. (Baby one was a girl, Baby 2 was a boy) We just had her closest friends, I think we had maybe 10 people, 15 max....I don't remember exactly. She already had all the big ticket baby stuff (stroller, carseat, swing, etc...) She mostly got a few outfits, diapers, a few boy toys, new pacis, and little things like that.
This exactly. A few of my friends threw me one for DD. There were like 5 friends there and they invited the husbands and kiddos too. It was a backyard party with appetizers. Super low key but still really special.
We have been on a "Sprinkle" for the second baby kick in my family. It is much smaller and more casual than a shower, and the gifts are also smaller. We tend to do items that need to be replaced or gender specific items. For my sister there were 10 of us, all family. For my SIL we are having lunch out and it will just be four of us. Both DH and I come from close families that really just like to have a reason to get together and celebrate. Both my sister and SIL sprinkles were/are surprises. Neither one asked for or expected anything.
