Cereal should never be added to the bottle unless suggested by the Dr. for acid reflux.
My mom tried to get me to give DS cereal in his bottle from about 3 months old. I never did it. We started solids at 4 months old - by spoon of course.
I often see young girls around here with what looks to me like carrot or s.potato puree mixed with milk in the bottle.
I won't be giving cereal until 5.5 months. It is not necessary unless recommended by the pedi. My DD has reflux and our pedi said we could use, but I didn't want her on any soilds. I just feel she is too young for it still. I start at 5.5 months spoon feeding a few tablespoons a day to get her used to the spoon so I can start purees at 6 months.
We will probably start with baby oatmeal around 5 months or whenever our pedi recommends. We started then with DD. We will feed by spoon not in the bottle.

We didn't use cereal.. we did oatmeal and with a spoon when she was 5.5 months... We JUST started stage 2/3 in the last week and she's 8 months old now
