  i think you have to mark it purchased on a wish list..if its an actual registry it does come off if it is the exact item.

  As long as they buy it off the wish list and it is an amazon item, it comes off. If the item was added by the "universal wish list button" then they have to take an extra set and mark that they purchased it else where.

  I think if they click to purchase the item through your wishlist.  I've had people buy stuff and it doesn't get marked as purchased, and several people end up buying the same item.
