Questions to as MFM tomorrow?

After my last OB appt, I left feeling a little uncomfortable with the plan of action moving forward...mostly no cervical length checks and no NSTs until 36 weeks so I plan to discuss both with MFM. I know he checks my cervical length at u/s but I wasn't going to be seeing him unless a need arose so I hope I can just ask that he take over the monthly u/s because otherwise I love my OB. Are there any other things I should cover with him while there?

  • I would definitely be concerned about the cervical length checks too. For me, in first tri until mid second tri they were every three weeks, and then every 2 after that.  They quit checking at about 34 weeks I think- everything was still looking fine.  For me, NSTs started with my OB at 33 weeks I think.  Also,when I got cervical length checks for the last 3 months or so, I was also getting BPPs and growth scans. This was all just routine, too- I have di/di twins and have had a fabulous pregnancy.  For months now I have had at least one appointment every week- either MFM or OB.  And over the past few weeks, when a BPP revealed a cord flow problem, as many as 3/ week. 

    It might sound like overkill, but I think frequent monitoring has been key for how well things are going.  When my cervix got down to 2.9, I was not put on bedrest, but really took it super easy after that, and have hung in there, working up until last Friday.  And because of the BPPs, we moved up my c-section date (breech baby) to Wednesday. If doctors aren't checking, how do we know it's all OK? I just feel like everyone is really taking good care of me and the babies, and it's helped me be as relaxed as I can be, given how much I worry about everything!

    Finally, if you don't feel comfortable with your OB, you might want to shop around.  It's not too late!  (Maybe ask the MFM if s/he thinks what your OB is doing is enough).

    Most MFMs are referral only, so they don't come up with the monitoring plan without an OBs involvement. I would ask though for sure. I had cervical checks every 3-4 weeks through 31 weeks and NSTs were scheduled for twice a week starting at 32 weeks (they started a week early due to IUGR, but 32 weeks was standard for any twin pregnancy). I would not have been comfortable with less monitoring, but I know many have less and are just fine.

    The MFM respects my OB but my OB has said that if he'd want to do the u/s there, she'd be fine with that and the MFM said the same thing. So, I figure if I ask, he may be willing to do so otherwise I may switch to the OB practice the MFM is a part of. I'd prefer not to switch because I really like my OB and otherwise think she's great, I just want to make sure they are doing everything they should be.

  • I love the MFM they are more confident I feel than the OB! I see my MFM as much as I see the OB, so double the appts. I only wish they delivered. I think the MFM will explain the whole process to you. GL
