Nanny Tax W2 Question

We hired a nanny this year, and I am trying to complete her W-2 for the deadline tomorrow.  I think the the boxes 1, 3, and 5 would be the same, but wanted to get a decond opinion??

    we have intuit payroll and on the one the program created, those numbers are the same for ours

    If you will pay her share of social security and medicare taxes (in addition to your share) on your tax return, then you add that amount to box 1 wages (but not 3 or 5). If you withheld SS and medicare taxes from her pay, then all 3 boxes would be the same.
