My boys' birthdays are also 3 weeks (2 yrs and 3 weeks, that is) apart. We've successfully done combined parties every year so far.
Not that your kids are in school yet, but I've noticed it helps keep the guest list limited to close/actual friends instead of every kid in class, which I like better personally. This was the first year that we tried to have a larger party and invite lots of kids from school -- almost none of them bothered to RSVP or commit one way or the other. It was quite frustrating, especially since we booked a party place. Next year we go back to close friends only.
We always do small parties. (My oldest 2 are also 3 weeks apart.) The exception will be big parties like this summer. We are thinking we will combine and invite a lot of kids this summer and do it early (before the end of the school year when everyone goes on vacation.)

I am actually thinking of doing a half birthday next summer. Birthdays are about celebrating the child, right? Doesn't matter WHEN. I want one outdoor, park, warm weather party for the boys, at least once.
