During Labor for pain relief would like to: Walk, Use birthing ball, Use Shower, Change Labor Positions, Distraction , Massage, Hot/Cold therapy, Going to bring my own music.
I want my husband in the room with me at all times and for him to cut the cord
I want to avoid vacuum and forceps usage
I want to try to avoid a Episiotomy and try a perineal massage
I want to breastfeed within the first hour
I don't want Briella to have a pacifier

Very nice. Short, sweet, and to the point. I bulleted mine so nurses can read it easily and not have to read paragraphs. I also put my Ob's name, practice, and phone number as well as our Ped's name, practice, and phone number.
Also it is obvious that you are going for a vaginal birth, but maybe mention your thoughts about a c-section in case it gets to that point.
I included our wishes for if things went far from our plan, even though there's no indication that any of these things are likely to happen.
C-section: I noted that I've never had surgery, so I'm terrified. I want my SO seated at my head, procedures/sensations explained, and to see baby asap.
Sick baby: SO is to follow baby to NICU/other hospital. I want to do as much kangaroo care & breastfeeding as possible. If baby can't take breast, I want to pump and feed.
Death of baby: we want to dress him, take footprints, photos and hold a funeral. recovery in a room as isolated from other L&D rooms as possible.
It's scary to think that things may go so far from our plan, especially since everything with this pg has been so normal and absolutely average. But I felt like discussing the worst case scenario would make it so that if it did happen, we'd be able to handle it a little bit better.
