My DD had reflux in the NICU (born at 34 weeks), and was on erythromycin and Pepcid for about six months. It was never really bad, probably because the meds worked well. She's been off the meds for about 2 months now, and has done fine with minimal spitting up and no eating-related fussiness.
However, she came down with a bad cold and cough on Sunday. She had fever a couple of nights ago, and yesterday, she refluxed several times after eating (on different occasions). I find that if I keep her upright after eating she does okay, like we used to have to do when she was a newborn.
Anyone else with reflux babies who notice that it gets worse when they're sick? Should I be worried / call the doctor? This happened before when she had a cold, but only once.

Same issue here! My LO has a bad cold and hacking cough. He has no fever, is eating fine, playing etc but very congested. He's been spitting up some, and I started getting worried that his solids were causing his reflux to flare up. However, the pediatrician said that it's just all related to post nasal drip and congestion. They don't know how to effectively expectorate mucous in their throat, so it gathers and chokes them up. My LO will chow down on his bottle and then get choked up and, burp and spit up some.
Try the Nosefrida! The suction is so strong that it can actually help pull some of the junk from their throat. Also, I try to be aggressive with the saline spray to wash the snot (that I can't suck out) down and encourage him to sneeze it out.
This is what our pedi said as well.
