THis is known as the "station" of placementof the baby, its based on the ischial spines (part of the pelvic bone, that can be felt when doing a vaginal examinations "-" stations refer to ABOVE the "spines" and "+" stations are below (or closer to being OUT!) some sites go from -5 to +5 others (liek my hospital) go grom -3 to +3 (just bigger increments, same distance overall obviously), as labour progress and baby descends it will get lower in the pelvis. O station gernerally means the baby is engeged in the pelvis and will not ascend or move back up
Here is an example using a "5" scale. I hope that was clear!

I'm curious, at what point can the baby's head be felt during an internal exam? DS floated way high until right before delivery. Seemed like he really decended in the hour right before pushing or so. I seriously thought I was having diarreah (sp?) on the table. He did have a nice round head though :0)

You can generally feel baby well before he or she is engaged in the pelvis, I mean its not THAT big of a passage (LOL). Often if baby is very high (not engaged) and the cervix is not yet well dilated the head is "bolatable" when we touch it it "bounces" up, baby is not in the pelvis (this would be unsafe to break waters as the cord could come down). AS the baby descends further and the cervix dilates more we feel more and more of the head. Also, if the cervix is very thick and posterior (back and not towards the front) it often isn't possible to feel the presenting part. But typically once labout has started the head is right there against the cervix, they typically don't have a whole lot of room left at the end!
