Anyone Pregnant again after a preterm delivery?

My OB suggested progesterone shots from 16 weeks until 37 weeks because my past baby was delivered at 36 weeks, 5 days.  I was so very close to full term but she said that one day shy makes me a "candidate" for the progesterone shots.

I'm wondering if people who were also rather close to full term decided to do the shots or not.  I'm trying to read the literature but am not sure whether the risks outweigh the benefits.  I wish there was more research done on the babies post delivery.

If anyone has any insight, I'm very interested!  TIA!

    I'm so afraid I'll feel guilty if I don't do them and then god forbid deliver really early.

    Then again, I feel guilty injecting myself with something weekly that my body doesn't need.  

    I feel like I'm in such a Catch 22 :(

  • I delivered during week 33 because of pre-e. My doctor did not say anything about progesterone. I've honestly never heard of that. She just wants to watch out for me getting pre-e though.

    That sounds crazy! DS was born 1 day short of full term and with my second LO this was not mentioned--i did have some additional testing done for preterm later on in the pregnancy and she was born at 38 weeks. I dont know if there are any down sides to progesterone shots but I would probably get a second opinion.

  • I went into PTL at 32 weeks... they stopped it using medicine and I delivered at 38 weeks. My doctor has never mentioned this...

  • P-17 is usually only used in women who delivered at less than 35 wks not 37. Did your child have complications after delivery or have to stay in the NICU for an extended time? I highly doubt your insurance would cover it because that is outside of the recommendations for it's use. I delivered at 32 wks because of PTL, so if I ever had another baby then I would be put on it. It is really only indicated with a history of PTL AND pre-term delivery. I am an OB nurse & I've never heard of it being used in that kind of scenario.

    It only helps if you deliver preterm, not go into preterm labor and stop the contractions.  That's probably why your doctor hasn't said anything.

    Technically I did have PTL AND pre-term delivery, since DS was born at 36.5.

    My child was healthy and did not stay in NICU.  He was almost 7lbs and delivery was a breeze (minus the tearing).

    I haven't read about the "less than 35 weeks" - do you have additional resources on where you found this...I'm very interested.  

    I plan to do my own research and thoroughly question my doctor at my next appointment.  I didn't even think about insurance, thanks for that thought!

  • I don't have any resources, just how I know it's being used here. I think most OB's just consider it unecessary to use it to prevent another pre-term birth if that delivery was @ 36+ wks, especially if the baby experienced no complications as a result of that delivery.

    17-p is ridiculously expensive & many insurance companies are not covering it even in situations where it is clearly needed. Any time you are considering a medication in pregnancy you have to weigh the pros/cons. Pros would be that it *may* allow you to go past 37 wks this time around. Cons would be cost, potential unecessary exposure to medication & tons of additional appts for weekly injections.

    ACOG covered patients for use of they had a prior delivery of less than 36.6, so technically you are in that group but would your outcome been any better had you made it to 37.0? Either way I think you'll be just fine but I would definitely get lots of opinions, especially if it is going to cost you & arm & a leg. Maybe ask to speak to a perinatologist regarding it? They are usually the ones who would make this decision anyways. Good luck & I hope your pregnancy is uneventful!
