Its an underwire and its padded. I wanted something I can wear now and after baby since I've already gone up 3 cup sizes Im going to get another from motherhood for the hospital and sleeping it looks like a sports bra but has padding, you lift it up from the bottom to nurse. It was super comfy, I just didnt have the money to buy both at the time.
The medela sleep bra's are for just sleeping (imo). they don't offer much support or coverage. For me, it was really just something to hold the nursing pads in case I leaked. I loved mine and wore them every night while I was nursing/pumping.
I didn't get any "real" nursing bras until after baby came and my milk was more established... they were $$ so I didnt want to waste money if nursing wasnt successful.
Also, I'd gone up several sizes while pregnant, then went up more in cup size due to Milk... but my inches around went down some after the baby. I just don't know if getting them before hand is practical since you won't really know what size you need.
I went to Target right after I got home from the hospital and got a couple of their $15 nursing tanks until I could get to the lactation store and get fitted for a good nursing bra.

Good idea on the sizing problem, I hadn't thought of that!! Maybe for now I will just get one of the sleep ones and some nursing tanks and wait on the regular nursing bra until I get a better idea of what size. Thanks!

I don't know if they have changed or what, but I hated the silouette of them and then I washed them - they became completely misshapen and I haven't purchased undergarments from there since
Target's Gillian O'Malley Nursing Bra - non- padded underwire bras were great b/c the shape wasn't bad, the price was right, and they were very supportive. GL!
I have issues with being "plus" size but not having "plus" size boobs. I ordered some from laurensilva.com and I love them. They are still a little bit big in the cup but I figure when my milk comes in they will fill out. I honestly don't remember what "style" they are but know that they are underwire and were pretty comfy. I also ordered a nursing tank from JC Penny...it's not the clasp kind, it just pulls to one side or the other. I liked it too.
The only one I've bought so far is the Medela sleep bra, and I love it! It's definitely not supportive enough to wear all day. I agree with PP who said it's great for holding nursing pads through the night. I'm going to wait on buying regular nursing bras because I have no idea what size I'll need!
I loved the pea in a pod nursing bra without underwire last baby. I would wait though to see what your chest does after the baby is born to know what size to get. Your rib cage normally gets smaller and your chest bigger. Just get a cheap one for at first...I like the target ones okay.
