Thrush and pedi says don't treat

I'm pretty sure I have trush. My baby is 5 months old and yesterday, out of nowhere, I started having severe piercing pain while BFing and my nipples looked sore too. This is after 4+ months of pain-free BFing. The main reason I am actually very sure that it is thrush is because my LO has been battling a yeast infection in his neck fold and diaper area for almost 2 months now, so there is a very good source of infection. The only thing is, I don't see any thrush in his mouth. I spoke to the pedi and he said that he could prescribe drops for me to use but would not want to treat my baby unless there is visible thrush in his mouth. Now everything I've read, and my common sense, says to treat baby and mother at the same time. Do I ignore the doctor's orders, or try it and see what happens?

  • How are you planning on treating it?  I was in a similar situation except without any symptoms besides the pain in nipples and am taking pro-biotics and using a nipple shield just in case.  I also did the Candida diet for 4-5 days (until I was having serious sugar withdrawal :).

  • You both need to be treated and Pedi's are useless when it comes to thrush. Talk to a good LC.  
  • I was given fluconaze or something like that. The 1 pill and I spoke to LOs pedi and he said to get 1% Gentian Violet. It's over the counter and only some pharmacies carry it. We found it at walgreens. Apply it to the tounge once a day for 2 days. Online it says 2-3 days and to put it on him then nurse so that the GV is also going onto your nipples. I'll be trying that method today. Yesterday I just put it on his tounge with a  q-tip. LO also has a really bad rash but i've just been caking on the vaseline.
  • You definitely both need to be treated! It can be so hard to get rid of and you just end up passing it back and forth. I'm loosely following Dr. Jack Newman's candida protocol and LO and I are both on diflucan (you need to take it for at least 2 weeks when you're dealing with nipple thrush). I'd push your pedi and share what you've read. It's miserable if you don't get it treated!

  • So my LO now actually has visible thrush so we are both definitely going to be treated. Problem is, the pedi prescribed Nystatin, which I know there is a lot of resistance for. My LO can't take Diflucan because of his heart condition. I started the Nystatin but tomorrow will ask if there is something else we can take.

    What else do I need to do- in terms of sterilizing pacifiers, toys, breast pumps, etc. How careful do I need to be?

  • OK, I'm going to disagree. If you don't see anything in LO's mouth, he doesn't need to be treated. I mean, treat the yeast he has, but you don't need to treat him for oral thrush. I had thrush and treated it before it spread to my baby-- it went away just fine. I spoke with a pediatric RN and an LC about it and that was their advice.

    I used OTC cream for athlete's foot on my nipples and just spent a lot of time topless. I was careful about washing anything that came in contact with my breasts or milk in hot water and sterilizing my pump, etc. I did not rewear bras or reuse burp clothes. I avoided sugary foods. It took about 2 weeks, and then we were all clear.

    Just be vigilant for signs in LO's mouth and treat them if you see them. 

  • I just spoke to two LCs about this very question. I told them that everything that I read said that mom and baby needed to be treated at the same time even if the baby was not showing symptoms. They told me that new research has come out in the past 3 years saying to only treat the person showing symptoms. I'm currently using a version of all purpose nipple cream (anti-fungal, anti-bacterial and hydro-cortisone) and believe that I have noticed some decrease in pain.
  • If you are not sure there is a very easy solution and it's not bad if it's not thrush!  Take grapefruit seed extract, put about 10 drops in an ounce of water, use a cotton ball and rub it on your nipples after you feed and use a qtip and rub it around your babies mouth after feeding.  It's natural and it really helps!  Do it after feeding and you both should be better in two days!
