I know there was a post the other day about how much to give for day care provider gifts. We just started the week after Thanksgiving, so I was thinking of giving half a week's pay split with 1/2 to the DCP and 1/4 each to her 2 assistants.
But when and how do you give it?
If you're giving cash or a cash equivalent -
When do you give it?
Do you include anything else (e.g., card or baked good)?
Is cash okay or should I do a gift card?

I think you're very generous to provide a holiday bonus if you've been there a month! A bonus goes a long way though IMO.
Cash is universally beloved.
We give our holiday bonus as an extra check inside our holiday card.
I do cash for two reasons.
1. Based on the two DC's DD has been to, I have noticed that some of the DC workers did not have cars. I didn't want to give them a giftcard to a place that is hard for them to get to on the bus/metro.
2. I know that from talking to a few of DD's workers at her former DCP that some of the workers were struggling to make ends meet. One woman I talked to had family somewhere in Africa and she said they didn't have money for food so she sent part of her paycheck to them each month. I didn't feel that it was appropriate to give her something like a Starbucks gift card so that she can go get a $5 coffee when her family can barely pay for food. I just figure that with cash, people have a choice of whether to use it to cover some of their basic needs or whether to do something special with it. Even if they use it to do something like buy groceries, I would think it is appreciated just as much (if not more)
Of course, all of this depends on your particular DC workers but for me, since I don't know some of them very well, I figure that cash is best.
So, what I do each year is I give them tins of cookies with a card taped to the top. The card has a handwritten letter and cash in it.
I also think you're very generous for your short time, but I think it will be very appreciated!
We've been using our DCP since August, and I'm always late, often ask her to work Fridays (norm is only Mon-Thurs), and she goes above and beyond for us in every possible way. I'm giving her 1 weeks pay on a Visa gift card from giftcards.com, so I could custom make it with a pic of DD on it. I also bought gifts for her two daughters (my DD is only child she watches - then her 2yo is there, and 5yo is at school). I plan to give it the last day she'll be there before Xmas (for me, thats the 22nd), and will give baked goods as well.
if your child is at a center, how much do you give? I feel really weird giving cash to a teacher in a center, it feels like I am bribing her to like my chid; I am going to do a gift. In fact, this morning DD said "I want to give Ms. teacher_name a present"
