Sorry all of my posts are about this pregnancy today. (Ahhh I am THAT girl)
I am supposed to write a letter to the hospital with my intended last day before going on my Mat leave. If I can, I would like to work right up until the week before my EDD (I know that is most likely a pipe dream) but they want an "intended last day"
Do I include something about having to go off early due to a sick leave? Or do I just clearly state that barring any "unforeseen circumstances" I intend to work until April 20, 2012.
Thoughts?

I would use the "unforseen circumstances" line and put whatever date you want.
If something changes, they will understand. And your doctor can always write a note or whatever if they give you any slack about it.
Yup. Just tell them what your plan is. Then if you go out early, you will probably just have to call them to let them know what happened (ex - my last day was supposed to be my induction day, but I had him the day before. So sometime the next week I called the benefits people to tell them my leave actually started on the 25th, not the 26th).
One thing that helped me was remembering that even though it's was my first time going on maternity leave, they did it for loads of people before me.

I planned on working up to both EDD. When I went early, my work didn't have any problems. With Lucas, I worked on Friday and on Monday went in for my scheduled c/s. Obviously he wasn't a twin, but we all assumed that I would be on bed rest. Thankfully that didn't happen though.
In your situation, I would go with the "barring any unforeseen circumstances, I intend to work until April 20.
