So I preregistered with the hospital and a couple weeks ago I received a packet to be filled out before we arrive. It's the "Mother's Worksheet for Child's Birth Certificate". Cool. One less thing to mess with once Butterbean gets here. But why does it have questions about my height, weight (pre-PG and at delivery), cigarette usage, educational background, WIC status, etc.? This seems like a LOT of information that the state doesn't need to know. Anyone know what's up with this questionnaire?

No clue but I can see why they might need weight but educational and WIC just seems like a demographic questionnaire thing they ask.
I would say if you are uncomfortable answering any of those then I wouldn't. If they really want to know then they can ask you while you are in the hospital. I have to say that I wouldn't answer any of those and if they really needed that then they can get the answer form my regular doc.
I got the same questionnaire, and haven't filled it out because it seems too intrusive. I figure I'll just deal with it at the hospital.
I got the same thing in the mail and just filled it out. I assume it's for the statistics and honestly, I don't care if they know if I am white, small or whatever else they asked.
I am giving up a lot more personal information for the process to immigrate into this country and am used to that kind of questions.
