O's Weight Gain Update - input welcome

i hate trying to xp without offending the bump gods:

So I know Ive been detailing you all the issues with O's weight gain. Here is the most recent update, and Id love to hear your thoughts:

After nearly wo months with minimal gain, the pedi had me fortify O's bottles of breastmilk with formula toi give him extra calories. I also took O to Crouse to see the LC to give me some feedback as to whether the latch was wrong or if something was going on during BFinfg that might answer my problems.

While I was nursing and after, as usual, O spit up. A lot. The LC at Crouse told me that our latch was good, but based on his spit up, it was pretty clear that she thought his weight gain issues had to do with the fact that he was not keeping the milk down... And she suggested that he could have reflux, or an allergy or worst case, pyloric stenosis.

Hearing this, I was pretty angry. I have mentioned O's spit up to the doc at EVERY VISIT, and during one visit O even spit up three times in front of the doc. But because he doesnt appear to be in pain when he spits up, the pedi just dismissed it as O being "a spitty baby"

Flash forward to Friday. At O's weight check, he gained 5 ounces in a week with the formula fortifying his milk, which was good... so the pedi was ready to leave it at that and just have me continue to fortify (which I will). But again, I broughtup the issues the LC mentioned, and he pretty much dismissed them again, saying that if I wanted, he would prescribe meds for reflux.

I dont want to just start medicating O without knowing exactly whats going on. Id really like him to see a gastro specialist to ensure that it is indeed reflux. Im also now seriously considering finding a new pedi.

What do you ladies think? Anyone with experience with reflux/allergies/etc?

  • I don't have any reflux advice, but if it were me, I would switch doctors.   There are so many good pediatricians in syr, you should have someone you feel comfortable with.

  • I agree with Emilie - I'd get a second opinion from another pedi. I wouldn't want to medicate him without being absolutely sure that that's what you're dealing with. Glad there was some weight gain this time around. Hope you find a solution soon.

  • I know that often reflux meds are given without seeing a specialist first.  It really is one of those - if it works then that is what it is.  I'm not dismissing your concerns with the pedi - bc you definitely should switch if you don't feel comfortable there or like you trust him.  Can you go to see a specialist without the pedi's rec?  Our insurance does not require a referral, so I have made appointments with a specialist with out input from the regular dr for dh's foot issue and my teeth problems lately.

  • Who does he see right now?  I would get a second opinion.  I hate feeling dismissed.
  • Sophia had reflux.  She was on meds for 9 months.  She was on the lowest dose of Zantac.  When I took her for her 1 month appt the NP said she has curdled milk at the back of her nose and she might have reflux.  Well we went to see my family later that week and she was miserable. I called the dr and wanted her put on something.  Within 24 hours she was more pleasant and the spit up decreased. 

    I would find a new ped.  I switched when Ethan was 3 months old b/c his dr made me feel stupid. It was the best decision.  I think he had reflux as well but it was blown off by the dr.

  • Does your insurance require a referral for a specialist? If not, I would make an appointment with a GI specialist or I'd be demanding one from my pediatrician. We did this when B was 2 months old b/c she had only gained 5oz in 6 weeks. She was screaming during most of her waking hours and her pediatrician attempted several different reflux meds. Come to find out it wasn't reflux. It was issues with her ability to digest lactose/galactose (the milk sugar which is in breast milk). When we finally started treatment, she was a completely different baby. Happy, cooing, smiling (non of which we got the first 3 months of her life).

    FWIW, I would get a new pediatrician as well. I hate being dismissed or feeling like doctors are on some high platform and you, as the parent, know nothing.

  • I would probably look into a new pedi.  If you really do think it's reflux, I would probably start meds and see if they help before visiting a gastro specialist.  Reflux meds for infants are pretty mild in terms of side effects, and everything I've read about testing infants for reflux is pretty invasive, like endoscopies.  I believe most diagnoses of reflux are made based on symptoms alone (and poor weight gain is defInitely a symptom).
  • I went through something really really similar when Eve was little. She just did not gain weight..and she spit up a lot. I actually had two different pedi's offices dismiss it. Once I started adding formula, she did start gaining weight.. but she continued to spit up a LOT (even after starting purees and cereal) until she was about 11 months. She was never medicated for reflux, but Natalie is.. the meds don't stop the actual spit up though, they just make it less painful for them.. so I don't know if they'd do much.

    That being said.. do you really wanna put the little guy through a swallow study, when it may or may not show anything?  From what I've heard from other reflux moms, they have to just sort of by luck catch them refluxing in the few minutes they're doing the test.. which doesn't always happen, even in kiddos with severe reflux.. and my pedi also said that 80% of babies under 6 months do have "reflux" but they aren't all in pain because of it.  I'm not sure of the diagnosis process for pyloric stenosis, but I think thats another test..

    If its an allergy, you could try elimination diets.. you could do that on your own easy enough without the pedi's OK.

    However, I stuck around with a pedi I wasn't 100% comfortable with for way too long with Eve, and if you don't feel like they're listening to you.. SWITCH.

  • Thanks ladies - I think I am going to look into a new pedi after O's 6month appt in a few weeks (I dont want him to get behind on his shots). Any SYR recs out there? Right now we are seeing Abbamont (and have also seen Levitt) at Pediatric Associates... I think that TinyPinkBug gave me a BFing friendly practice before, but I dont remember the name... Any other recs?

     Melissa - yea - putting O though the tests is something Im concerned about as well... but Id also hate to just blanket put him on meds too... I honestly dont think its pyloric stenosis though Ive also thought about doing the elimination diet - the LC sugested I go full force and eat just rice, lamb, pears (and sweet potatoes, squash and oatmeal, since thats what O is eating), for a few weeks to see if that helps...

    I also just bought an electronic scale, so I want to start doing some weighed feedings as well.

  • We go to dr magowan at Northcourt pediatrics. She's been very BF friendly and a great doc so far...
