Trying to switch to Tricare Standard

I'm trying to switch to Tricare Standard but of course the Tricare Website isn't making it easy. Does anyone know where to find the form that needs to be filled out (without going to the MTF)?

    Yes, go to the forms section and click on TriCare Prime Disenrollment Form.  You'll be able to print it out from there.
