DOR/Poor Responders - 3dt or 5dt?

I'm getting ahead of myself here and I may come to regret it.  Whether I need this information now or another cycle in the future, it's still something I'm thinking about today.

I'm reading that for DOR and poor responders, you're more likely to get a 3dt.  I'm also seeing that 5dt tend to be used more where there is a higher number (something like more than 4?) of day 3 embryos.   

What are your thoughts on this, or what has your experience been?  I know it's largely out of our hands -- I'm just trying to understand it all.   It looks like in a case like mine, we'll almost certainly do a 3dt -- assuming we have anything to transfer at all.

  • I don't completely understand the whole 3dt versus 5dt issue so I don't know how much help I will be but I did ask that question of my former RE and he claimed that the only reason to do a 5dt is if you have a high number of good quality embryos at day 3. The extra 2 days allows them to really narrow it down to the best of the best. I have read so many other things though so I don't know how accurate or all-inclusive that explanation really is. As for me, I am getting a 6dt. As part of the clinical trial I am in, I am guessing that the extra days allow them to do the full CCS/PGD work-up that the trial is based on. True, it is mostly out of hands if not completely but stay positive!
    Me: 31, Dx: Hypothalamic Amenorrhea, Elevated AMH of 18 DH: 34 & borderline morphology issues IUI#1/cycle with Menopur & Novarel = Mild OHSS & BFN IUI#2/cycle with Menopur & Novarel = BFN IVF#1 converted to FET due to moderate OHSS 21 eggs retrieved, 12 mature, 11 fertilized 7 frosties, 6 genetically normal December 5th FET with one perfect blast = BFN FET #2 = early February

  • Thanks bluefish.  What you said is consistent with my limited understanding, too.  The part I forgot to add, but which you mentioned, is that 5dt are typically used when there is a high number of high-quality 3 day embryos.  So, considering I have, oh 3 follicles right now (with the promise of 1 or 2 more), I'd definitely be in the 3dt category :-)  

    This whole process is amazing.  It truly is because there's still so much magic and miracle to it, despite the extremely clinical and seemingly controlled process.

  • If you have a good # that are good quality my clinic will do a 5dt. If there are a few that are ok, my clinic will do a 3dt. If you only got say 2, they will do a 2dt, or 3 with some not being great quality it will be a 2dt.

    Thanks Lynn.  I didn't know that a 2dt is an option depending upon the circumstances.  I know I won't have a good number, and quality is an unknown at this point.

  • My Re said more than 3 a 3dt and less than 3 a 2dt.  Last time, I had five that fertilized and 3 of good enough quality to transfer.  They did a 3dt.  We will see what happens this time.
  • I have had a 2dt both times due to only having 2 embies. We were definitely putting both back so why wait and risk them arresting and have nothing to transfer.

  • I had a 3dt because we only had one embryo - the doc didn't even consider a 5dt - if we had waited until day 5 its possible our one embryo wouldn't have made it. I had 3 follicles retrieved, 2 were mature eggs, only one fertilized successfully (with ICSI). The one embryo stuck and I'm 25 wks PG.
